摘要
目的探讨重复经颅直流电刺激治疗帕金森(PD)患者的效果及对躯干肌电特征、步行功能及生存质量的影响。方法选取荆门市第二人民医院收治的PD患者221例,根据治疗方法不同分为试验组111例和对照组110例,两组均给予基础的药物治疗措施,试验组同时给予重复经颅直流电刺激治疗;对比两组患者治疗前后帕金森评分量表(UPDRS)评分、三维步态分析时空参数、行走时躯干肌表面肌电振幅及生存质量评分。结果治疗前,试验组和对照组的UPDRSⅠ评分、UPDRSⅡ评分、UPDRSⅢ评分及UPDRS总分差异无统计学意义(P>0.05);治疗后,试验组患者的UPDRSⅠ评分、UPDRSⅡ评分、UPDRSⅢ评分及UPDRS总分均低于对照组,差异有统计学意义(P<0.05)。治疗前,试验组和对照组的步速、步频、步宽结果差异无统计学意义(P>0.05);治疗后,试验组患者的步频、步宽结果大于对照组,差异有统计学意义(P<0.05)。治疗前,试验组和对照组的胸竖脊肌、腰竖脊肌、腹外斜肌、腹直肌表面肌电振幅测定值差异无统计学意义(P>0.05);治疗后,试验组患者的胸竖脊肌、腹外斜肌表面肌电振幅低于对照组,差异有统计学意义(P<0.05)。治疗前,试验组和对照组的PDQ-39评分差异无统计学意义(P>0.05);治疗后,试验组患者的PDQ-39评分低于对照组,差异有统计学意义(P<0.05)。结论重复经颅直流电刺激联合基础疗法治疗PD患者能有效改善患者的躯干肌电水平、步行功能及生存质量。
Objective To explore the effect of repeated transcranial direct current stimulation in the treatment of Parkinson's(PD)patients and its influence on trunk electromyographic characteristics,walking function and quality of life.Methods According to the treatment methods,221 PD patients were divided into 111 experimental group and 110 control group,Both groups were given basic drug treatment measures,and the experimental group was given repeated transcranial direct current stimulation treatment at the same time;Before and after treatment,the patients had Parkinson Rating Scale(UPDRS)scores,three-dimensional gait analysis time and space parameters,trunk muscle surface electromyography amplitude,and quality of life scores during walking.Results Before treatment,the UPDRS I score,UPDRS II score,UPDRS III score and UPDRS total score of the experimental group and the control group were not statistically different(P>0.05);after treatment,the UPDRS I score,UPDRS II score,UPDRS III score and UPDRS total score of the experimental group The scores were lower than those of the control group(P<0.05);before treatment,there was no significant difference between the test group and the control group in stride speed,stride frequency,and step width(P>0.05);after treatment,the stride frequency of the test group The result of step width was larger than that of the control group(P<0.05);before treatment,there was no significant difference in the measured values of surface electromyographic amplitude of the thoracic erector spinae,lumbar erector spinae,external oblique muscles,and rectus abdominis between the test group and the control group Significance(P>0.05);After treatment,the surface EMG amplitudes of the thoracic erector and external oblique muscles of the test group were lower than those of the control group(P<0.05);before treatment,the PDQ-39 scores of the test group and the control group The difference was not statistically significant(P>0.05);after treatment,the PDQ-39 score of the test group was lower than that of the control group(P<0.05).Conclusion Repetitive transcranial direct current stimulation combined with basic therapy to treat PD patients can effectively improve the patient's trunk myoelectric level,walking function and quality of life.
