应用德尔菲法构建中国城市地区慢病相关社区效能评估体系 被引量:12

Establishing the evaluation system of community efficacy for non-communicable diseases in Chinese urban area by Delphi method

摘要 目的基于社区赋能理论和慢病防控相关因素构建关于"慢病相关社区效能"的评估体系,用于评估城市社区慢病防控情况,改善和提高居民健康水平。方法应用德尔菲专家征询法,邀请来自国内外30名社区卫生、慢病管理和健康促进等领域资深专家和实践者,开展了两轮专家咨询,结合李克特五级评分制法、百分制分配法和重要性排序法进行问卷设计,并开展了专家深度访谈,对"慢病相关社区效能"评估体系进一步筛选和优化。结果第1轮专家积极性系数为0.80,第2轮为0.85,专家权威系数均高于0.80。在第2轮德尔菲法后,得到了5个一级指标,分别是社区物质环境、慢病行为危险因素、心理与人际关系、社区卫生管理和社区组织与活动。其下属共18项二级指标和71道问题条目。绝大部分二级指标的重要性得分标准差小于10,变异系数小于0.3。结论 "慢病相关社区效能"评估体系可系统地对社区慢病防控能力进行诊断和评估,以支持社区慢病防控政策的制定和相关活动的推行。 Objective To establish an evaluation system of "Community Efficacy for Non-communicable Diseases(NCD)"(COEN)on the basis of community empowerment theory and related factors for prevention and control of NCD,in order to evaluate the prevention and control of NCD in Chinese urban communities,and to improve the health of residents.Methods The present study adopted the Delphi study method by inviting 30 Chinese and international experts and practitioners in the fields of community health,NCD management,and health promotion,and completed two rounds of Delphi surveys.The Delphi surveys combined the5-level Likert scale,100-point scale,and the ranking scale in the questionnaire design,and was further supported by in-depth interviews to finalize the COEN evaluation system.Results The participation rates of the present study were 0.80 and 0.85 in the first and second round of the Delphi survey,respectively.The authority indicators of involved experts were all above 0.80 in the related fields.The second round of Delphi survey yielded a revised COEN evaluation system that consisted of 5 first-level dimensions,which were community physical environment,NCD behavioral risk factors,mental health and social relationships,community health management,and community organizations and activities.They further included 18 second-level indicators and a total of 71 items.The scores of relevance for most second-level indicators had a standard deviation lower than 10,and the coefficients of variation were lower than 0.3.Conclusion The COEN evaluation system could be used to evaluate the communities’ abilities to prevent and control NCD for the residents,and to support the related policy making and activity planning in urban China.
