摘要
目的:探讨腹腔镜输卵管开窗取胚术对异位妊娠患者术后输卵管再通及受孕的影响。方法:选择2017年1月-2019年5月接诊的86例异位妊娠患者,根据随机数字表法分为对照组与观察组,每组43例。观察组行腹腔镜输卵管开窗取胚术治疗,对照组行开腹输卵管开窗取胚术治疗。比较两组术中、术后情况及受孕率和输卵管再通率。结果:观察组手术操作时间短于对照组,术中出血量小于对照组,差异有统计学意义(P<0.05);观察组下床活动时间、肛门排气时间、腹痛消失时间、留置导尿管时间、住院时间均短于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05);观察组术后总受孕率、宫内妊娠率、输卵管再通率高于对照组,差异有统计学意义(P<0.05)。结论:腹腔镜输卵管开窗取胚术治疗异位妊娠患者具有创伤小、并发症少、恢复快等优点,能提高术后输卵管再通率和受孕率。
Objective:To investigate the effect of laparoscopic fallopian tube fenestration and embryo extraction on postoperative fallopian tube recanalization and pregnancy in patients with ectopic pregnancy.Methods:A total of 86 patients with ectopic pregnancy admitted to our hospital from January 2017 to May 2019 were selected and divided into two groups according to random number table method,with 43 cases in each group.Observation group was treated with laparoscopic fallopian tube fenestration,and the control group was treated with open fallopian tube fenestration.The intraoperative and postoperative conditions,pregnancy rate and fallopian tube recanalization rate were compared between the two groups.Results:The operation time of the observation group was shorter than the control group,and the intraoperative blood loss was lower than the control group.The difference was statistically significant(P<0.05).The observation group's time to get out of bed,anal exhaust time,abdominal pain disappearance time,indwelling catheterization time,and hospital stay were shorter than those of the control group,and the difference was statistically significant(P<0.05).The postoperative complication rate in the observation group was lower than the control group.The difference was statistically significant.The internal pregnancy rate and fallopian tube recanalization rate were higher than those of the control group,the difference was statistically significant(P<0.05).Conclusion:Laparoscopic tubal fenestration and embryo extraction for treatment of ectopic pregnancy has the advantages of less trauma,fewer complications,and faster recovery.It can improve the postoperative fallopian tube recanalization rate and pregnancy rate.
