基于大脑分割技术的头颅MRI多模态联合血清Aβ在轻度认知功能障碍中的特点 被引量:2

摘要 目的大脑分割技术运用于多模式MRI影像结合血清Aβ和P-tau,探讨影像学和血清学指标在轻度认知功能障碍(MCI)中的特点。方法119例65岁的老年患者被评估诊断为MCI(MCI组55例)和正常老人(NC组64例)。所有患者经头颅MRI扫描(包括T1、DTI、ASL、DWI序列),同时血清Aβ1-42、Aβ1-40和P-tau检测。头颅MRI序列经过MNI软件处理将所有脑区分割为116个设定的脑区。两组均行分割区域的MR指标和血清Aβ1-42、Aβ1-40和P-tau比较,分析MRI指标(DTI的FA值,ASL的CBF值,DWI的ADC值)和血清学指标在MCI中的特点以及MRI指标与血清学的关系。结果血清学方面,MCI组的血清Aβ1-40水平高于NC组(P<0.05),而Aβ1-42和P-tau在两组间无差异(P>0.05);影像学方面,4个区域(右侧罗兰迪克岛盖、右侧小脑脚、右侧小脑3区、左侧小脑4~6区)的FA值两组有显著差异,在右侧罗兰迪克岛盖、右侧小脑脚1区和左侧小脑4~6区三个区域MCI组高于NC组,在右侧小脑3区区域低于NC组(P<0.05);22个区域的ADC值两组有差异(P<0.05);没有发现任何区域的脑血流量(CBF值)在两组中有差异(P>0.05)。回归分析发现血清Aβ1-40与左侧小脑4~6区的FA线性相关(P<0.05)。结论血清Aβ1-40是MCI的敏感标志物,其结合小脑的FA值异常升高可更好的对MCI早期诊断;血清Aβ1-40与小脑白质纤维损害正相关。 Objective Brain segmentation technique was applied to multimode MRI imaging combined with serum Aβlevels and P-tau to investigate the characteristics of imaging and serum indicators in mild cognitive impairment(MCI).Methods A total of 119 elderly patients aged 65 years were diagnosed as MCI(55 in MCI group)and normal elderly(64 in NC group).All patients underwent cranial MR scan(including Tl,DTI,ASL and DWI sequences),and serum Aβ1-42,Aβ1-40 and p-tau were detected simultaneously.The cranial MR sequence was processed by MNI software to divide all brain regions into 116 designated brain regions.In both groups,the MR index of the segmentation region and serum Aβ1-42,Aβ1-40 and P-tau were compared,and the characteristics of MR index(FA value of DTI,CBF value of ASL and ADC value of DWI)and serum index in MCI were analyzed,as well as the relationship between MR index and serum index.Results Serum Aβ1-40 levels in MCI group were higher than those in NC group(P<0.05),while Aβ1-42 levels and P-tau had no difference between the two groups(P>0.05).FA of four brain sections(right Rolandic operculum,right cerebelum Crusl,left Cerebelum4-6,right cerebelum3)in the two groups have significant differences,on right Rolandic operculum,right Cerebelum Crusl and the left cerebellum 4-6,FA in MCI group is higher than the NC,FA of the right Cerebelum3 lower than NC group(P<0.05);ADC values of 22 regions were different between the two groups(P<0.05).There was no difference in CBF values in any region between the two groups(P>0.05).Regression analysis showed that Aβ1-40 was linearly correlated with FA of sections of left Cerebelum4-6(P<0.05).Conclusion Serum Aβ1-40 was sensitive marker of MCI,and combined it with increased FA of cerebellum can facilitate early diagnosis of MCI.Serum Aβ1-40 was positively correlated with fiber damage of cerebellum white matte.
