摘要
目的探讨心肌梗死患者PCI术后超声心动图后二尖瓣舒张早期血流峰值速度与二尖瓣环舒张早期运动峰值速度的比值E/E’与其心功能变化的关系。方法选取中山市坦洲人民医院2018年1月至2020年11月收治的62例行PCI术治疗的心肌梗死患者作为研究对象,另取30例同期年龄性别组成匹配的正常体检者作为对照组,患者术前、术后1 d、术后1个月、术后3个月与对照组体检当日均采用超声心动图测定心功能指标,包括左心室舒张末内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)、左心室收缩末期容积指数(LVESVI)、舒张末期容积指数(LVEDVI)、室壁运动积分指数(WMSI),计算E/E’比值,比较心肌梗死患者不同时间点与对照组的E/E’与其他心功能指标的关系。结果心肌梗死组患者术前LVESD、LVESVI、LVEDVI、WMSI与E/E’均高于对照组,LVEDD、LVEF低于对照组,差异均有统计学意义(P<0.05);术后1 dE/E’较术前有所降低,LVEF较术前有所升高,术后1个月与3个月LVEDD、LVESD、LVESVI、LVEDVI、WMSI、E/E’均较术前有所下降,而LVEF较术前有所升高,差异均有统计学意义(P<0.05)。在术前、术后1 d、术后1个月及术后3个月四个不同时间点检测的E/E’与LVEDD、LVESD、LVESVI、LVEDVI、WMSI均呈正相关,与LVEF均呈负相关,差异均有统计学意义(P<0.05)。结论心肌梗死患者PCI术后E/E’改善较快,表明PCI术后短期内舒张功能改善较显著,E/E’与心功能各项指标相关性稳定,能较好地反映PCI术后心功能的改变。
Objective To investigate the relationship between the ratio of the early diastolic flow peak velocity of mitral valve to the early diastolic peak velocity of mitral valve annulus(E/E')on echocardiogram and the changes of cardiac function in patients with myocardial infarction after PCI.Methods A total of 62 patients with myocardial infarction treated by PCI who were admitted to Tanzhou People's Hospital of Zhongshan City from January 2018 to November 2020 were selected as the research subjects,and 30 subjects of matching age and gender with normal physical examination during the same period were selected as the control group.Echocardiography was used to determine the cardiac function indexes of patients before operation,1 day after operation,1 month after operation,3 months after operation,and on the day of physical examination for the control group,including left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD),left ventricular ejection fraction(LVEF),left ventricular end-systolic volume index(LVESVI),left ventricular end-diastolic volume index(LVEDVI),wall motion score index(WMSI).The E/E’ratio was calculated and the relationship between E/E’and other cardiac function indexes at different time points was compared between patients with myocardial infarction and the control group.Results The preoperative LVESD,LVESVI,LVEDVI,WMSI and E/E’of the myocardial infarction group were higher than those of the control group,and the LVEDD and LVEF were lower than those of the control group,both with statistically significant differences(P<0.05);the E/E'on the first day after operation was lower than that before operation,and LVEF was higher than that before operation.LVEDD,LVESD,LVESVI,LVEDVI,WMSI and E/E'were all decreased at 1 month and 3 months after operation,while LVEF was higher than that before operation.The differences were statistically significant(P<0.05).E/E'detected at four different time points,before operation,1 day after operation,1 month after operation,and 3 months after operation,showed a positive correlation with LVEDD,LVESD,LVESVI,LVEDVI,and WMSI,and a negative correlation with LVEF,with differences all statistically significant(P<0.05).Conclusion The E/E'of patients with myocardial infarction improves faster after PCI,indicating that the diastolic function strengthens significantly in the short term after PCI,and the correlation between E/E'and various indexes of cardiac function is stable,which can better reflect the change of cardiac function after PCI.
