经阴道超声预测卵泡发育评估卵巢储备的价值

摘要 目的探讨经阴道超声预测卵泡发育评估卵巢储备功能的价值。方法选取2019年5月—2020年7月期间我院治疗的74例不孕症患者作为观察组,同期在我院体检的健康者70例作为对照组,两组均开展经阴道超声检查,比较两组性激素水平,以及观察组患者在自然周期和促排卵周期的子宫内膜厚度、收缩期峰值流速与舒张末期血流速比值(S/D)和阻力指数(RI),再根据激素检查结果将其分为卵巢功能正常组(n=34)和卵巢功能下降组(n=40),比较两组患者窦卵泡数(AFC)和卵巢动脉血流动力学指标。结果对照组性激素水平中卵泡刺激素(FSH)水平低于观察组,雌二醇(E2)水平高于对照组(P<0.05),两组促黄体生成素(LH)水平差异统计学意义(P>0.05);观察组自然周期子宫内膜厚度低于促排卵周期,RI及S/D高于促排卵周期,差异有统计学意义(P<0.05);卵巢功能正常组患者AFC多于卵巢功能下降组,差异有统计学意义(P<0.05),RI和卵巢搏动指数(PI)低于卵巢功能下降组,收缩期峰值血流速(PSV)高于卵巢功能下降组,差异有统计学意义(P<0.05)。结论经阴道超声对卵泡发育有良好的预测功能,同时还可评估卵巢储备功能,有利于及时确定不孕病因并开展合理治疗,故值得推广应用。 Objective To explore the value of transvaginal ultrasound in predicting follicular development and evaluating ovarian reserve.Methods A total of 74 infertility patients admitted to our hospital from May 2019 to July 2020 were selected as the observation group,and 70 healthy patients who received physical examination in our hospital during the same period were selected as the control group.Both groups were examined by transvaginal ultrasound.Compare the levels of sex hormones between the two groups,as well as the endometrial thickness,peak systolic flow rate to end-diastolic blood flow rate(S/D)and resistance index(RI)of the observation group during natural cycles and ovulation induction cycles,and then check according to hormones.Results The level of follicle stimulating hormone(FSH)in the control group was lower than that in the observation group,and the level of estradiol(E2)was higher than that in the control group(P<0.05).There was significant difference in the level of luteinizing hormone(LH)between the two groups(P>0.05);In the observation group,the thickness of endometrium in natural cycle was lower than that in ovulation induction cycle,and RI and S/D were higher than that in ovulation induction cycle(P<0.05);AFC in patients with normal ovarian function was more than that in patients with decreased ovarian function(P<0.05),RI and ovarian pulsatility index(PI)were lower than those in patients with decreased ovarian function,and peak systolic blood flow velocity(PSV)was higher than those in patients with decreased ovarian function(P<0.05).Conclusion Transvaginal ultrasound has a good predictive function for follicular development,and it can also assess ovarian reserve function.It is helpful to determine the cause of infertility in time and carry out reasonable treatment,so it is worthy of popularization and application.
