摘要
目的:研究妊娠早期甲状腺功能筛查意义及妊娠期亚临床甲减患者的治疗效果。方法:选取1062例进行例行产检的早孕孕妇进行甲状腺功能筛查,确诊妊娠期亚临床甲减患者56例。按随机数字表法分对照组与研究组,各28例。对照组孕妇未遵医嘱用药,研究组孕妇口服左甲状腺素钠片,两组均观察至分娩结束。结果:对照组孕妇血清TPOAb、TSH水平比研究组降低;对照组孕妇血清FT4水平比研究组升高;妊娠期间研究组并发症发生率与对照组相比明显降低;研究组孕妇不良妊娠结局发生率与对照组相比明显降低(均P<0.05)。结论:妊娠早期进行甲状腺功能筛查,可早期发现妊娠期亚临床甲减,通过及时有效的左甲状腺素钠片治疗,可减少妊娠期亚临床甲减患者妊娠期并发症及不良妊娠结局的发生率。
Objective:To study the significance of thyroid function screening in early pregnancy and the effect of treatment in patients with subclinical hypothyroidism during pregnancy.Methods:A total of 1062 earlypregnancy pregnant women who underwent routine check-ups were selected for thyroid function screening,and 56 cases of subclinical hypothyroidism during pregnancy were diagnosed.According to the random number table,the control group and the study group were divided into 28 cases.The control group did not follow the doctor's prescription,and the study group took levothyroxine sodium tablets.Both groups were observed until the end of delivery.Results:The serum TPOAb and TSH levels in the control group were lower than those in the study group after treatment;the serum FT4 levels in the control group were higher than those in the study group after treatment;the incidence of complications in the study group during pregnancy was significantly lower than that in the control group the incidence of adverse pregnancy outcomes in the study group was significantly lower than that in the control group(all P<0.05).Conclusion:Early pregnancy screening of thyroid function can detect subclinical hypothyroidism in pregnancy early,and timely and effective treatment with levothyroxine sodium tablets can reduce the incidence of pregnancy complications and adverse pregnancy outcomes in patients with subclinical hypothyroidism during pregnancy.
