摘要
目的探讨经皮导板Chevron截骨治疗轻中度外翻的近期疗效。方法回顾性分析了济南市人民医院手足外科于2018年1月至2020年1月收治的12例轻中度外翻患者,所有患者均采用经皮导板Chevron截骨治疗,分析治疗后的临床疗效及并发症发生情况。结果治疗后,本组12例患者的第一跖趾关节跖骨间角(hallux valgus angle,HVA)、第1、2跖骨间角(inter metatarsal angle,IMA)均优于治疗前(P<0.01);治疗后,患者的视觉模拟评分法(visual analogue scale,VAS)评分、(the American Orthopaedic Foot and Ankle Society,AOFAS)踝-后足评分均优于治疗前(P<0.01);患者的治疗满意率达到91.67%;第一跖骨矫形后长度平均短缩1.34 mm,但矫形前后的跖骨长度差异无统计学意义(P>0.05);无伤口感染、皮缘坏死、不愈合等并发症发生。结论经皮导板Chevron截骨治疗外翻,术中截骨精确,可微创纠正外翻畸形,操作安全,疗效可靠。
Objective To evaluate the short-term effect of percutaneous guided Chevron osteotomy in the treatment of mild to moderate hallux valgus.Methods 12 patients with mild to moderate hallux valgus in Jinan City People’s Hospital from January 2018 to January 2020,were analyzed retrospectively.All patients were treated with percutaneous guided Chevron osteotomy.The clinical efficacy and complications after treatment were analyzed.Results After treatment,the HVA and IMA of 12 patients were better than before treatment (P<0.01);After treatment,the VAS score and AOFAS score were better than before treatment (P<0.01);The treatment satisfaction rate of shortened patients reached 91.67%;The length of the first metatarsal bone after operation was shortened by 1.34 mm on average compared with that before operation;There was no significant difference in metatarsal length before and after operation (P>0.05);All patients had no complications such as wound infection and metatarsal head necrosis.Conclusion The application of guide Chevron percutaneous osteotomy in the treatment of hallux valgus can achieve precise intraoperative osteotomy,correct valgus deformity minimally and relieve pain.The operation safety and reliability are relatively high.
