摘要
冠心病后多合并焦虑障碍,两者虽属不同疾病,但均可表现为"心悸、胸闷、心烦、多汗、失眠"等核心症状,李平教授将中医学整体观"运用到双心病诊疗中,抓住心血管病交感神经兴奋"症状背后对应的核心病机,基于"郁在中焦"和"心主血脉-神明论",提出冠心病患者素有脾虚,的基础脾虚生痰,痰浊直接损伤"血脉之心"影响"心主血脉"功能;在不良情绪刺激下,痰郁化热,痰蔽神窍,热损脑络,进一步上扰"神明之心"影响"心主藏神"功能,因此认为在双心病的发生发展过程中共有"痰热"病机。治疗时从"痰热"论治重视调理气机,以清心化痰,解郁安神治疗收效满意。
Coronary heart disease(CHD) is often complicated with anxiety disorder.Although they are different diseases,they can be manifested as " palpitation,chest tightness,upset,sweating,insomnia’ and other core symptoms.Professor LI Ping applies the holistic view of TCM to the diagnosis and treatment of double heart disease,seizing the corresponding core pathogenesis behind the sympathetic excitement symptoms of cardiovascular events proposing that CHD patients always have spleen deficiency based on " depression in the middle energizer" and "heart dominating blood vessels-mental activities" theory.Spleen deficiency generates phlegm and directly damages the "heart of blood vessels" and affects the function of "heart dominating blood vessels";under the stimulation of bad emotions,phlegm stagnates to heat,phlegm covers the mind orifices,heat damages the brain collaterals,further disturbs" the heart of mental activities" and affects the function of "the heart storing spirit".Therefore,it is considered that there is a common pathogenesis of "phlegm heat" in the occurrence and development of double heart disease.During the treatment,Prof.LI pays attention to regulating qi clearing heat and resolving phlegm,relieving depression and calming the nerves,and gets satisfactory results.
