摘要
目的:研究中医推拿松解肩胛下肌对冻结期肩周炎患者肩关节疼痛、活动度和功能的即刻疗效。方法:将70例冻结期肩周炎患者用随机数字表法分为对照组35例、试验组35例,均予常规物理治疗和肩关节推拿治疗,试验组在此基础上再予以中医推拿手法松解肩胛下肌。治疗结束后即对疗效进行比较,测量治疗前后关节活动度(外展和后伸)、VAS疼痛评分,以及肩关节功能改善情况,并进行评定。结果:治疗前,两组患者肩关节活动度(AROM)、VAS评分及肩关节功能总评分差异无统计学意义(P>0.05)。两组治疗后AROM比较,试验组显著高于对照组(P<0.05),试验组VAS评分低于对照组(P<0.05),试验组肩部活动功能总评分高于对照组(P<0.05)。结论:松解肩胛下肌能够明显缓解冻结期肩周炎患者的肩部疼痛、增加关节活动度、提高肩部活动功能,具有显著的即刻效应,值得临床推广应用。
Objective:To explore the immediate effect of releasing the subscapularis muscle on pain,range of motion and function in patients with frozen scapulohumeral periarthritis by Chinese Tuina therapy.Methods:A total of 70 patients with frozen scapulohumeral periarthritis were randomly divided into control group(n=35)and treatment group(n=35).Both groups were treated with infrared ray for 20 min and massage to release infrascapular muscle and circular muscle for 20 min.Chinese Tuina therapy on subscapularis muscle was added to the treatment group.Immediately after the end of treatment,the efficacy was compared,the range of motion(abduction and extension)was measured,the pain was assessed using the visual analogue scale(VAS),and the improvement of shoulder joint function was assessed using the Functional Assessment Program for Rehabilitation and Physical Therapy for Scapulohumeral Periarthritis scale comparing to that before the treatment.Results:Before treatment,there was no significant difference in shoulder range of motion(AROM),visual analogue scale(VAS)and total shoulder function score between the two groups(P>0.05),with comparability.The shoulder range of motion(AROM)of the two groups after treatment was significantly higher in the test group than in the control group(P<0.05),the value of the shoulder visual analogue scale(VAS)test group was lower than that in the control group(P<0.05),and the total shoulder range of motion score in the test group was higher than that in the control group(P<0.05).Conclusion:Releasing the subscapularis muscle can significantly relieve the shoulder pain of patients with frozen periarthritis of shoulder,increase the joint range of motion of patients with frozen periarthritis of shoulder,and improve the shoulder activity function of patients with frozen periarthritis of shoulder,with significant immediate effect.It is worthy of being widely popularized for clinical application.
