摘要
目的探讨单光子发射计算机断层成像术(SPECT)/CT检查对早期甲状腺癌(DTC)术后颈部淋巴结转移的诊断价值。方法回顾性分析2018年1月到2019年12月112例在本院行DTC根治术患者的病例资料。分析患者术后SPECT/CT检查结果,与病理结果进行比较,分析两者对DTC术后颈部淋巴结转移的判断价值。结果以术后病理学诊断为“金标准”,112例DTC术后患者存在46例(41.07%)颈部淋巴结转移。回顾性分析患者SPECT/CT检查结果后,将其与病理检测结果进行比较,发现漏诊13例,误诊20例。SPECT/CT诊断颈部转移灶的灵敏度、特异度和准确度分别为71.74%、69.70%、70.54%;阳性预测值和阴性预测值分别为62.26%、77.97%。结论SPECT/CT可以对DTC术后颈部淋巴结转移提供较为准确的判断,为早期术后治疗提供参考。
Objective To explore the diagnostic value of single-photon emission computed tomography(SPECT)/CT in cervical lymph node metastasis after differentiated thyroid cancer(DTC).Methods A retrospective analysis was performed on case data of 112 patients who underwent the radical operation of DTC in the hospital from January 2018 to December 2019.The results of postoperative SPECT/CT were analyzed.Moreover,they were compared with pathological results.The determining value of the two for cervical lymph node metastasis after DTC was analyzed.Results Taking postoperative pathological diagnosis as the“golden standard”,among 112 patients after DTC surgery,there were 46(41.07%)cases with cervical lymph node metastases.After retrospective analysis of SPECT/CT examination results,they were compared with results of the pathological examination.13 cases were missing the diagnosis,and 20 cases with misdiagnosis.The sensitivity,specificity,and accuracy of SPECT/CT in diagnosing cervical metastases lesions were 71.74%,69.70%,and 70.54%,respectively.Its positive predictive value and negative predictive values were 62.26%and 77.97%,respectively.Conclusion SPECT/CT can more accurately determine cervical lymph node metastasis after DTC surgery,providing early postoperative treatment.
