摘要
目的:研究在不同气候因素下穴位敷贴对肺肾两虚型慢性阻塞性肺疾病(COPD)稳定期Ⅱ-Ⅲ级患者肺通气功能及生活质量的影响。方法:收集2019年4月至2020年8月中医科收治的肺肾两虚型COPD患者150例,采用随机分组、平行对照法,分为治疗1组、治疗2组和治疗3组各50例。治疗1组在常规治疗基础上在三伏天加用穴位敷贴,治疗2组在伏天过后秋季加用穴位敷贴治疗,治疗3组在三伏天和秋季持续治疗。观察三组治疗前后肺通气功能、临床中医症状、6 min步行距离(6MWD)和生活质量评分等指标的变化。结果:治疗1组的总有效率为76.0%(12/50),治疗2组为54.0%(23/50),治疗3组为84.0%(8/50),组间差异有统计学意义(P<0.05)。治疗后,治疗3组的中医临床症状总积分改善程度与其他两组相比差异有统计学意义(P<0.05)。与治疗前相比,治疗后三组患者的肺通气功能指标均得到改善(P<0.05),且治疗3组优于其他两组(P<0.05)。治疗3组患者6MWD改善程度与其他两组相比差异有统计学意义(P<0.01)。治疗3组的生活质量评估与其他两组相比差异有统计学意义(P<0.05)。结论:穴位敷贴配合常规治疗肺肾两虚型COPD患者在三伏天和秋季持续治疗具有更加良好的临床疗效,并能改善生活质量。
Objective:To study the effects of acupoint application on pulmonary ventilation function and quality of life in stable stageⅡ-Ⅲpatients with chronic obstructive pulmonary disease(COPD)of deficiency of lung and kidney under different climatic factors.Methods:One hundred and fifty patients with COPD with deficiency of lung and kidney treated in the traditional Chinese medicine department from April 2019 to August 2020 were collected,and using randomized grouping and parallel control method,divided into a treatment group 1,a treatment group 2 and a treatment group 3 with 50 cases each.The treatment group 1 was treated with acupoint application on the basis of routine treatment in dog days,the treatment group 2 was treated with acupoint application in autumn after dog days,and the treatment group 3 was treated continuously in dog days and autumn.The changes of pulmonary ventilation function,clinical symptoms of traditional Chinese medicine,6-minute walking distance(6MWD)and quality of life scores before and after treatment in the three groups were observed.Results:The total effective rate was 76.0%(12/50)in the treatment group 1,54.0%(23/50)in the treatment group 2 and 84.0%(8/50)in the treatment group 3 and the difference between the groups was statistically significant(P<0.05).After treatment,the improvement of total score of TCM clinical symptoms in the treatment group 3 was statistically significant compared with the other two groups(P<0.05).Compared with before treatment,the indexes of pulmonary ventilation function in the three groups were improved after treatment(P<0.05),and the treatment group 3 was better than the other two groups(P<0.05).Compared with the other two groups,the improvement of 6MWD of the treatment group 3 was significantly different(P<0.01).The quality of life in the treatment group 3 was significantly different from that in the other two groups(P<0.05).Conclusion:Acupoint application combined with conventional treatment of COPD patients with deficiency of lung and kidney has a better clinical effect in dog days and autumn continuous treatment,and can improve the quality of life.
