摘要
目的探讨小视野扩散加权成像对前列腺癌的诊断价值。方法回顾收集83例经临床病理诊断为前列腺癌的患者,所有病例术前行MRI常规视野扩散加权成像与小视野扩散加权成像检查以及实验室检查。由两位医师对图像进行主观评分并测量病灶的ADC值。MRI影像资料结合PSA检查结果、病理结果进行统计学分析。结果r-FOV DWI的主观图像质量显著高于常规c-FOV DWI(P<0.05)。r-FOV DWI测得的前列腺病灶的ADC值低于c-FOV DWI,但差异无统计学意义(P>0.001)。c-FOV DWI组选取ADC值为1.074×10^(-3) s/mm^(2)时,其诊断前列腺癌的灵敏度、特异度分别为81%和63%。在r-FOV DWI组选取ADC值为0.993×10^(-3) s/mm^(2)时,其诊断前列腺癌的灵敏度、特异度分别为79%和75%.两种诊断方法的ROC曲线下面积分别为0.813和0.825,鉴别诊断前列腺癌的诊断效能相当(P>0.05)。结论小视野扩散加权成像比常规视野扩散加权成像具有更好的图像质量,能为前列腺癌的诊断提供更有效的方法。
Objective To investigate the value of reduced field-of-view diffusion-weighted imaging(r-FOV DWI)in diagnosing prostate cancer.Methods We retrospectively selected 83 patients with a pathological diagnosis of prostate cancer.All the patients had undergone conventional-FOV DWI(c-FOV DWI)and r-FOV DWI before surgery.Two doctors scored the images on a subjective basis and measured the apparent diffusion coefficient(ADC)of the lesions.MRI data,prostate-specific antigen results,and pathological results were analyzed.Results The subjective image quality of r-FOV DWI was significantly higher than that of c-FOV DWI(P<0.05).The ADC value of lesions measured on r-FOV DWI was lower than that on c-FOV DWI,but there was no significant difference(P>0.001).When ADC was 1.074×10^(-3) s/mm^(2) in the c-FOV DWI group,the sensitivity and specificity of diagnosis were 81%and 63%,respectively.When ADC was 0.993×10^(-3) s/mm^(2) in the r-FOV DWI group,the sensitivity and specificity were 79%and 75%,respectively.The areas under the receiver operating characteristic curves of the two methods were 0.813 and 0.825,respectively,indicating comparable diagnostic performance for prostate cancer(P>0.05).Conclusion r-FOV DWI has better image quality than c-FOV DWI,and can serve as a more effective approach to the diagnosis of prostate cancer.
