摘要
目的浅析在“互联网+药品流通”背景下,药店电子处方的用药现状与合理性。方法遵循随机抽样法,选择该地区某几大网络医疗平台,回顾性分析2020年全年6000张已于药店完成购药的电子处方单(500张/月),核算不合理处方发生率与种类。结果6000张电子处方单中,平均处方药品数1.62种,抗菌药物处方占比62.30%,国家基本药物使用率71.78%,通用名使用率99.95%;处方单涉及的疾病中,发生率最高的是呼吸系统疾病(36.45%);所有处方中不合理处方占4.62%,其中不规范处方占1.75%,不适宜处方占2.87%。电子处方社会药店购药服务开通后购药耗时满意度评分、药师服务态度满意度评分、满意度总分均高于开通前,医药沟通便利性满意度评分低于开通前,差异有统计学意义(P<0.05);开通前后处方评审专业性满意度比较,差异无统计学意义(P>0.05);不良反应中胃肠道损伤的发生率最高,开通前后不良反应总发生率比较,差异无统计学意义(P>0.05)。结论电子处方社会药店购药模式下,处方合理性理想,因此患者用药安全性有保障,患者可以更加便捷地购买药品,并获得更加热情的药事服务。
Objective To analyze the current situation and rationality of the use of electronic prescriptions in pharmacies under the background of"Internet+drug circulation".Methods Follow the random sampling method,selected a few major online medical platforms in the region,retrospectively analyze 6000 electronic prescription orders(500 copies/month)that have completed the purchase of drugs in pharmacies throughout 2020,and calculate the incidence and types of unreasonable prescriptions.Results Among the 6000 electronic prescriptions,the average number of prescription drugs was 1.62,the prescriptions of antibacterial drugs accounted for 62.30%,the use rate of national essential drugs was 71.78%,and the use rate of generic names was 99.95%.Among the diseases involved in the prescriptions,the highest incidence was breathing systemic diseases(36.45%);irrational prescriptions accounted for 4.62%of all prescriptions,of which irregular prescriptions accounted for 1.75%,and unsuitable prescriptions accounted for 2.87%.After the opening of the electronic prescription social pharmacy drug purchase service,the satisfaction score of drug purchase time,the satisfaction score of the pharmacist's service attitude,and the total satisfaction score were all higher than before the opening,and the satisfaction score of the convenience of medical communication was lower than that before the opening,and the difference was statistically significant(P<0.05);comparison of professional satisfaction of prescription review before and after opening,the difference was not statistically significant(P>0.05);among the adverse reactions,the incidence of gastrointestinal injury was the highest,and the total incidence of adverse reactions before and after opening was compared,and the difference was not statistically significant(P>0.05).Conclusion Under the electronic prescription social pharmacy drug purchase model,the prescription is reasonable,so the safety of patients'medication is guaranteed,and patients can purchase drugs more conveniently and receive more enthusiastic pharmacy services.
