CPAP呼吸机治疗2型糖尿病合并阻塞性睡眠呼吸暂停综合征对血糖控制的疗效分析

摘要 目的分析呼吸机治疗2型糖尿病合并阻塞性睡眠呼吸暂停综合征(OSAHS)患者的临床疗效。方法纳入该院2018年5月—2021年4月收治的74例2型糖尿病合并OSAHS作为研究主体,以治疗方案不同分组,予以常规治疗患者归入对比组(n=37)、加用呼吸机治疗患者记作研究组(n=37),比较两组患者的临床疗效。结果治疗前,两组血糖水平、睡眠呼吸情况、血管内皮功能以及氧化应激情况均差异无统计学意义(P>0.05),治疗后,研究组空腹血糖(5.16±1.01)mmol/L、餐后2 h血糖(7.46±1.32)mmol/L、糖化血红蛋白(6.14±1.06)%、呼吸暂停低通气指数(6.88±4.32)次/h、最长呼吸暂停时间(11.56±23.64)s、最低脉搏容积血氧饱和度(84.17±22.45)%优于对比组,差异有统计学意义(P<0.05);治疗后,研究组空腹C肽(0.89±0.38)μg/L、餐后2 h C肽(1.69±0.74)μg/L均低于对比组,空腹胰岛素(26.33±6.55)mIU/L、餐后2 h胰岛素(64.51±12.69)mIU/L均高于对比组,差异有统计学意义(P<0.05)。结论在2型糖尿病合并OSAHS患者中,采取CPAP呼吸机治疗,能够更好控制血糖,改善睡眠呼吸情况、血清C肽及血清胰岛素水平。 Objective To analyze the clinical efficacy of ventilator in the treatment of type 2 diabetes with obstructive sleep apnea syndrome(OSAHS).Methods Included in the hospital from May 2018 to April 202174 cases of type 2 diabetes with OSAHS were treated as the main body of the study.The treatment plan was divided into different groups,and the patients who received conventional treatment were classified into the comparison group(n=37),patients treated with a ventilator were recorded as the study group(n=37).Compare the clinical efficacy of the two groups of patients.Results Before treatment,there was no statistically significant difference in blood glucose levels,sleep breathing,vascular endothelial function,and oxidative stress between the two groups(P>0.05).After treatment,the study group had fasting blood glucose(5.16±1.01)mmol/L,postprandial 2 h blood glucose(7.46±1.32)mmol/L,glycosylated hemoglobin(6.14±1.06)%,apnea-hypopnea index(6.88±4.32)times/h,longest apnea time(11.56±23.64)s,lowest pulse volume The blood oxygen saturation(84.17±22.45)%were better than that of the control group,and the difference was statistically significant(P<0.05);after treatment,the study group had fasting C peptide(0.89±0.38)μg/L,postprandial 2 h C peptide(1.69±0.74)μg/L were lower than the comparison group,fasting insulin(26.33±6.55)mIU/L and 2 h postprandial insulin(64.51±12.69)mIU/L were higher than the comparison group,the difference was statistically significant(P<0.05).Conclusion In patients with type 2 diabetes and OSAHS,CPAP ventilator treatment can better control blood glucose,improve sleep breathing,serum C-peptide and serum insulin levels.
