细胞因子对肾移植患者术后发生感染的预测价值与分析

摘要 目的:探讨肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、白细胞介素-1β(IL-1β)、白细胞介素-2(IL-2)、白细胞介素-8(IL-8)对肾移植患者术后发生感染的预测价值。方法:回顾性选取2017年12月-2020年12月在本院行同种异体肾移植手术的82例患者,根据感染诊断标准和培养结果将其分为感染组(n=42)和非感染组(n=40)。比较感染组与非感染组白细胞计数(WBC)、中性粒细胞计数(NEU)、TNF-α、IL-6、IL-10、IL-1β、IL-2、IL-8的水平差异,应用多因素logistic回归分析肾移植患者术后发生感染的独立影响因素,应用受试者工作特征(ROC)曲线分析细胞因子对感染的预测价值。结果:感染组TNF-α、IL-2均高于非感染组(P<0.05)。多因素logistic回归分析结果显示,IL-2为影响肾移植术后感染的独立危险因素(P<0.05)。ROC曲线分析结果显示,IL-2对肾移植术后感染预测价值的约登指数为0.682,曲线下面积为0.811。结论:IL-2为肾移植术后感染的独立危险因素,对肾移植术后发生感染有一定的预测价值。 Objective:To investigate the predictive value of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-10(IL-10),interleukin-1β(IL-1β),interleukin-2(IL-2)and interleukin-8(IL-8)for infection in renal transplantation patients.Method:From December 2017 to December 2020,82 patients with allogeneic kidney transplantation in our hospital were selected,according to the diagnostic criteria and culture results of infection,they were divided into the infected group(n=42)and in non-infected group(n=40).The white blood cell(WBC),neutrophil(NEU),TNF-α,IL-6,IL-10,IL-1β,IL-2 and IL-8 in the infected group and the non-infected group were compared,the influencing factors of infection after renal transplantation were analyzed by multivariate logistic regression analysis,ROC curve was used to analyze the predictive value of cytokines for infection.Result:TNF-αand IL-2 in infected group were higher than those in non-infected group(P<0.05).Multivariate logistic regression analysis showed that IL-2 was an independent risk factor for infection after renal transplantation(P<0.05).ROC curve analysis showed that Yuden index of IL-2 for predicting infection after renal transplantation was 0.682,the areas under the curve was 0.811.Conclusion:IL-2 is an independent risk factor for infection after kidney transplantation,and has certain predictive value for infection after kidney transplantation.
