摘要
目的了解脑脊液寡克隆区带在神经系统和脑血管相关疾病或症状中的分布特点和临床意义。方法对北京天坛医院2014年至2020年包括23种神经系统和脑血管相关疾病或症状,共4514例脑脊液寡克隆区带电泳结果进行回顾性分析。数据采用SPSS 20.0软件处理,以P<0.05为差异具有统计学意义。结果在不同疾病或症状中,脑脊液特异寡克隆区带的阳性率差异具有统计学意义(P<0.001),以多发性硬化症、脑囊虫病、脱髓鞘病、病毒性脑炎和颅内感染等的阳性率最高;以颅内静脉窦血栓、格林巴利综合征、肢体无力、周围神经病和痴呆最低。在不同疾病或症状中,脑脊液寡克隆区带电泳结果的分类差异具有统计学意义(P<0.001)。不同疾病或症状含有源自血清渗透寡克隆区带的比例差异具有统计学意义(P=0.001),其中脑血管病、格林巴利综合征、周围神经病、脑梗死和视神经脊髓炎有较高比例血清渗透来源于寡克隆区带。结论在不同神经系统和脑血管相关疾病或症状中,脑脊液特异寡克隆区带具有不同程度的阳性率,且寡克隆区带的分布具有差异。脑脊液寡克隆区带电泳检测对疾病的诊断、治疗具有一定的提示作用。
Objective To study the distribution and clinical significance of cerebrospinal fluid oligoclonal bands in patients with neurological disorders and cerebrovascular related disease or symptoms.Methods A retrospective analysis was performed on a total of 4514 electrophoresis samples of cerebrospinal fluid oligoclonal bands collected at Beijing Tiantan Hospital between 2014 and 2020,which represented 23 neurological disorders and cerebrovascular related disease or symptoms.Data were analyzed by software SPSS20.P<0.05 was considered statistically significant.Results Among different diseases or symptoms,the positive rate of cerebrospinal fluid special oligoclonal bands showed significant differences(P<0.001).The highest positive rates were identified in multiple sclerosis,cerebral cysticercosis,demyelinating disease viral encephalitis and intracranial infection,while the lowest in intracranial venous sinus thrombosis,Guillain-Barre syndrome,limbs weakness,peripheral neuropathy and dementia.The distribution of cerebrospinal fluid oligoclonal bands electrophoresis results showed significant differences among different diseases or symptoms(P<0.001).The proportion of oligoclonal bands derived from serum in different diseases or symptoms was significantly different(P=0.001),with higher proportion in cerebrovascular diseases,Guillain-Barre syndrome,peripheral neuropathy,cerebral infarction and neuromyelitis optica.Conclusion In different neurological disorders and cerebrovascular related diseases or symptoms,cerebrospinal fluid special oligoclonal bands show various degrees of positive rates,and the proportion of oligoclonal bands is different.The cerebrospinal fluid oligoclonal bands electrophoresis examination could facilitate the diagnosis and treatment of diseases.
