摘要
目的:观察重复经颅磁刺激(rTMS)对不完全性脊髓损伤(SCI)患者下肢肌肉痉挛及功能的影响。方法:选取2019年2月~2020年2月本院收治的SCI患者50例,按照随机数字表法分为治疗组和对照组各25例,对照组接受常规康复训练配合假rTMS刺激,观察组则在常规康复治疗基础上配合rTMS治疗,采用"8"字形线圈rTMS刺激不完全性脊髓损伤患者(M1区),刺激强度为80%的RMT,刺激频率9Hz,共4周,观察其对患者下肢改良Ashworth痉挛评分(MAS)的影响,并采用脊髓功能独立性评估量表Ⅲ(Spinal Cord Independence Messure,SCIM-Ⅲ)评估其功能。结果:治疗4周后,治疗组下肢MAS分级较对照组显著改善(P<0.05),治疗组SCIM-Ⅲ较对照组显著提高(P<0.05)。结论:rTMS治疗后SCI患者MAS和SCIM-Ⅲ指标变化较对照组改善显著,提示rTMS对脊髓损伤患者下肢痉挛有缓解作用,能够改善患者的肢体功能,值得临床推广应用。
Objective:To observe the effect of repetitive transcranial magnetic stimulation(rTMS) on lower limb muscle spasm and function in patients with incomplete spinal cord injury(SCI).Methods:Select 50 SCI patients admitted to our hospital from February 2019 to February 2020.According to the random number table,they were divided into treatment group(n=25) and control group(n=25).The control group received conventional rehabilitation training combined with fake rTMS stimulation,while observation group received rTMS treatment on the basis of conventional rehabilitation treatment.Patients in both groups were treated for 4 weeks.The "8"-shaped coil rTMS was used to stimulate patients with incomplete SCI(Ml area),the stimulation intensity was 80% RMT,and the stimulation frequency was 9 Hz for a total of 4 weeks.Spinal Cord Independence Messure(SCIM-Ⅲ) assesses its function.Results:After 4 weeks of treatment,the MAS classification of lower limbs in treatment group was significantly improved compared with control group(P<0.05),and the SCIM-Ⅲ of treatment group was significantly improved compared with control group(P<0.05).Conclusion;The changes of MAS and S CIM-Ⅲ indexes of SCI patients before and after rTMS treatment are significantly improved compared with control group,suggesting that rTMS can relieve lower limb spasticity in patients with spinal cord injury and can improve the limb function of patients,which is worthy of clinical application.
