摘要
目的探讨帕金森病(PD)合并直立性头晕患者的病因及其临床特点。方法收集2018年9月至2019年2月在海南医学院第二附属医院神经内科就诊的PD患者一般人口学信息及临床资料、帕金森综合评分量表(UPDRS)Ⅲ、Hoehn-Yahr(H&Y)分期量表及帕金森病自主神经功能障碍(SCOPA-AUT)量表。完善卧立位经颅多普勒(TCD)检测,根据卧立位试验心率、血压、脑血流速度的变化将患者进行病因分类。结果共收集87例PD患者,其中男性65例(74.7%)、女性22例(25.3%),年龄:(63.02±8.16)岁,病程:(5.41±4.0)年,起病年龄:(58.31±8.74)岁。其中30例(34.5%)人有直立性头晕,与PD不伴直立性头晕(PD-OD)患者相比,合并直立性头晕(PD+OD)患者年龄更大,UPDRS-Ⅲ评分更高,H&Y分级更高。依据卧立位TCD结果,PD+OD患者中主要包括直立性低血压伴直立性脑低灌注(OH-U)11例(36.7%)及直立性脑低灌注7例(23.3%),人数均高于PD-OD组(P=0.00)。结论PD合并有直立性头晕与神经循环异常有关,且可能与疾病进展相关。
Objective To explore the etiology and clinical characteristics of PD patients with orthostatic dizziness.Methods General clinical data were collected.Unified Parkinson’s Disease Rating Scale(UPDRS-III),Hoehn&Yahr staging、Scales for Out-comes in Parkinson’s disease-Autonomic(SCOPA-AUT)were obtained.Supine to standing TCD test were performed on all the patients,and patients were classified according to blood pressure,heart rate and cerebral hemodynamics recorded during supine to standing TCD test.Results A total of 87 PD patients were enrolled,including 65(74.7%)males and 22(25.3%)females.30 patients(34.5%)were orthostatic dizziness,with higher scores of UPDRS-III and H&Y stage,and older than PD-OD(PD patients without orthostatic dizziness)(P<0.05).According to the characteristics of blood pressure,heart rate and cerebral hemodynamics during active standing TCD test,PD+OD(PD patients with orthostatic dizziness)included 11(36.7%)OH-U(orthostatic hypotension-uncompensated),7(23.3%)orthostatic cerebral hypoperfusion,which was higher than in PD-OD group.Conclusions PD patients with orthostatic dizziness is related to neurocirculatory abnormalities,and may be associated with disease progression.
