摘要
目的研究血清黄体生成素(LH)基础值联合抗苗勒氏管激素(AMH)检测对女童中枢性性早熟(CPP)的诊断价值。方法选取2017年9月至2020年10月西安航天总医院收治的95例CPP女童为CPP组,单纯性乳房发育(PT)女童85例为PT组,同期选取本院体检健康女童100例为对照组。对CPP、PT组女童进行促性腺激素释放激素(GnRH)激发试验;罗氏Cobas e 602全自动免疫电化学发光分析仪检测所有受试者基础雌二醇(E2)、LH、卵泡雌激素(FSH)基础值及血清AMH水平。采用Pearson相关性分析血清LH基础值、AMH与CPP组女童GnRH实验参数的关系;采用接受者操作特性曲线(ROC)分析血清LH基础值联合AMH检测对女童CPP的诊断价值。结果与对照组比较,PT组女童的子宫体积、卵巢体积、血清E2水平、LH基础值、FSH基础值、AMH均显著升高,CPP组女童的身高、体质量、骨龄、子宫体积、卵巢体积、血清E2水平、LH基础值、FSH基础值、AMH均显著升高,差异均有统计学意义(P<0.05);与PT组比较,CPP组女童的身高、体质量、骨龄、子宫体积、卵巢体积、血清E2水平、LH基础值、LH峰值、FSH基础值、FSH峰值、LH峰值/FSH峰值、AMH均显著升高,差异均有统计学意义(P<0.05);CPP组女童的血清LH基础值和AMH水平与LH峰值和LH峰值/FSH峰值呈正相关(P<0.05);血清LH基础值、AMH诊断女童CPP的曲线下面积(AUC)分别为0.754(0.681~0.828)、0.774(0.705~0.843);两者联合诊断女童CPP的AUC为0.889(0.842~0.935),二者联合对女童CPP的诊断价值高于两者单独诊断价值,差异均有统计学意义(P<0.05)。结论血清LH基础值联合AMH检测对女童CPP具有较高的诊断价值,可以为临床上CPP的诊断提供参考依据。
Objective To study the diagnostic value of serum luteinizing hormone(LH)basic value combined with anti-Müllerian hormone(AMH)detection for central precocious puberty(CPP)in girls.Methods From September 2017 to October 2020,95 CPP girls admitted to Xi’an Aerospace General Hospital were selected as the CPP group,85 girls with simple breast development(PT)were selected as the PT group,and 100 healthy girls were selected as the control group during the same period.Gonadotropin releasing hormone(GnRH)provocation test was performed on girls in CPP and PT groups.Roche Cobase 602 automatic immunochemiluminescence analyzer was used to detect all subjects’basal estradiol(E2),LH,follicular estrogen(FSH)basal levels and AMH levels.Pearson correlation was used to analyze the relationship between serum LH basal value,AMH and GnRH experimental parameters of girls in CPP group.Receiver operating characteristic curve(ROC)curve was used to analyze the serum LH basal value combined with AMH detection for the diagnostic value of CPP in girls.Results Compared with the control group,the uterine volume,ovarian volume,serum E2 level,LH basic value,FSH basic value,and AMH of girls in PT group were significantly increased,and the height,body mass,bone age,uterine volume,ovarian volume,serum E2 level,LH basic value,FSH basic value and AMH of girls in CPP group were significantly increased,with statistically significant difference(P<0.05).Compared with the PT group,the height,body mass,bone age,uterine volume,ovarian volume,serum E2 level,LH basic value,LH peak value,FSH basic value,FSH peak value,LH peak value/FSH peak value,and AMH of girls in CPP group were all significantly increased(P<0.05).Serum LH basal value and AMH level of girls in CPP group were positively correlated with LH peak value and LH peak value/FSH peak value(P<0.05).The area under the curve(AUC)of serum LH basal value and AMH for the diagnosis of CPP in girls was 0.754(0.681-0.828)and 0.774(0.705-0.843),respectively;the AUC of the two combined diagnosis of CPP in girls was 0.889(0.842-0.935),and the diagnostic value of the combination of the two was higher than that of the two alone,with statistically significant difference(P<0.05).Conclusion The combination of serum LH basal value combined with AMH detection has a high diagnostic value for CPP in girls,and may provide a reference for the clinical diagnosis of CPP.
