高频超声鉴别诊断桥本甲状腺炎与甲状腺功能亢进症的临床应用 被引量:4

摘要 目的分析高频超声鉴别诊断桥本甲状腺炎与甲状腺功能亢进症的临床应用效果。方法选取河南科技大学第一附属医院2019年10月至2020年10月收治的桥本甲状腺炎及甲状腺功能亢进患者各64例作为研究对象,分别为桥本甲状腺炎组与甲亢组,对两组患者均进行高频超声检查,观察比较两组患者甲状腺大小、甲状腺内回声、甲状腺结节、甲状腺内部有无血流改变及颈部淋巴结变化及其甲状腺内收缩期峰值流速、舒张末期流速、血流阻力指数及收缩期峰值到达时间等指标差异。结果桥本甲状腺炎组与甲亢组患者中在甲状腺大小、甲状腺内回声、甲状腺结节、甲状腺内部血流改变及颈部淋巴结变化中比较差异均有统计学意义(P<0.05);桥本甲状腺炎组患者收缩期峰值流速、舒张末期峰值流速均低于甲亢组患者(P<0.05),血流阻力指数及收缩期峰值到达时间高于甲亢组患者(P<0.05)。结论高频超声鉴别诊断桥本甲状腺炎及甲状腺功能亢进症中可有效反映患者甲状腺组织结构与生理变化情况,为临床诊断提供依据。 【Objective】 To analyze the clinical application effect of high-frequency ultrasound in the differential diagnosis of Hashimoto’s thyroiditis and hyperthyroidism.【Methods】 Each 64 patients with Hashimoto’s thyroiditis or patients with hyperthyroidism who were treated in the hospital between October 2019 and October 2020 were selected as the research subjects,and they were set as Hashimoto’s thyroiditis group and hyperthyroidism group respectively.Both groups of patients received highfrequency ultrasound examination.The thyroid size,echo within thyroid,thyroid nodules,presence or absence of blood flow changes within thyroid,changes of cervical lymph nodes,and peak systolic flow velocity,end-diastolic flow velocity and blood flow resistance index within thyroid and peak systolic arrival time were observed and compared between the two groups.【Results】 There were statistically significant differences in thyroid size,echo within thyroid,thyroid nodules,changes of blood flow within the thyroid and cervical lymph nodes changes between Hashimoto’s thyroiditis group and hyperthyroidism group(P<0.05).The peak systolic flow velocity and end-diastolic peak flow velocity in Hashimoto’s thyroiditis group were slower than those in hyperthyroidism group(P<0.05),and the blood flow resistance index and peak systolic arrival time were higher or longer than those in hyperthyroidism group(P<0.05).【Conclusion】 High-frequency ultrasound in the differential diagnosis of Hashimoto’s thyroiditis and hyperthyroidism can effectively reflect the thyroid tissue structure and physiological changes,and provide a basis for clinical diagnosis.
