摘要
目的总结方朝晖教授应用中药治疗原发性甲状腺功能减退症(原发性甲减)肝肾阴虚证的临证经验。方法将方朝晖教授采用中药治疗的原发性甲减肝肾阴虚证患者基线资料及处方信息录入临床科研一体化系统,通过关联规则、频数分析等方法分析中药的核心药物、关联组合、使用频次等。结果纳入了277例患者及其处方,女性占92.8%,平均年龄约40岁,含药218种;中药使用频次超过40%的有炙甘草、当归、茯苓、茯神、合欢花、远志、白芍;复杂网络图显示炙甘草、当归、茯神、合欢花、茯苓、远志、女贞子、生地黄8种药物组成核心处方;中药二药关联规则中设置信度>70%,支持度>10%的有4个,如红花-桃仁、女贞子-墨旱莲等;三药关联中置信度>80%,支持度>10%的有8个,如红花-桃仁-当归、茯神-茯苓-当归等。结论基于数据挖掘技术方法较好地总结了方朝晖教授重视滋养肝肾,兼活血、宁神的中药治疗原发性甲减肝肾阴虚证的学术思想。
Objective To summarize Professor FANG Zhaohui’s clinical experience in the treatment of liver and kidney yin deficiency syndrome of primary hypothyroidism. Methods The baseline data and prescription information of patients with liver and kidney yin deficiency syndrome of primary hypothyroidism treated by Professor FANG Zhaohui were entered into the integrated clinical research system. The core drugs, association combination and use frequency of traditional Chinese medicine were analyzed by association rules and frequency analysis. Results 277 patients and their prescriptions were included. Women accounted for 92.8%, with an average age of about 40 years and 218 kinds of traditional Chinese medicines. More than 40% of traditional Chinese medicines were roasted Licorice, Angelica, Poria cocos, Poria, Albizzia, Polygala tenuifolia and Radix Paeoniae Alba. The complex network diagram showed that the core prescription was composed of 8 drugs:Roasted licorice, angelica, Poria, Albizzia, Poria cocos, Polygala tenuifolia, privet fruit and Rehmannia glutinosa.In the association rules of traditional Chinese medicine, there were four with reliability >70% and support >10%,such as safflower, peach kernel, privet, Eclipta, etc. Among the three drug associations, 8 had confidence >80% and support >10%, such as safflower, peach kernel, Angelica sinensis, Poria God, Poria cocos, Angelica sinensis, etc.Conclusion Based on data mining technology, this paper summarizes Professor FANG Zhaohui’s academic thought of treating liver and kidney yin deficiency syndrome of primary hypothyroidism with traditional Chinese medicine that attaches importance to nourishing liver and kidney, promoting blood circulation and calming the mind.
