摘要
目的评价ORSIM支气管模拟器用于麻醉科住院医师规范化培训学员在纤维支气管镜引导气管插管术教学中的效果。方法选取2017-09/2019-09期间在我院麻醉科进行住院医师规范化培训的40名学员作为研究对象,将其随机分为对照组(C组,n=20)和试验组(E组,n=20)。C组采用理论授课加传统纤支镜气管插管模型学习,E组采用理论授课加ORSIM支气管模拟器学习。比较2组学员理论考核成绩及操作考核过程中2组学员的插管操作时间、碰壁次数、旋转角度总和、一次插管成功率和操作考试通过率。对2组学员进行问卷调查和自信心自我评价并进行比较。结果两组学员在理论考试中选择题、简答题及总分的平均成绩比较差异无统计学意义(P>0.05)。E组学员在插管时间、旋转角度总和明显低于C组(P<0.05),E组一次插管成功率和操作考试通过率均高于C组(P<0.05)。两组学员问卷调查中,在有助于增强学习兴趣、有助于增加教学亲和力、有助于提升实际操作能力及是否支持教学模式4项结果中,E组均明显优于C组(P<0.05);在学员操作自信心自我评价中,E组学员操作自信心明显高于C组(P<0.05)。结论使用ORSIM支气管模拟器教学具有较高的成功率和趣味性,能提高住院医师的主动钻研能力、临床基本技能及自信心,对麻醉科住院医师规范化培训学员在纤支镜引导气管插管术的教学中具有显著优势。
Objective To evaluate the effect of ORSIM bronchial simulator in fiberoptic bronchoscope guided endotracheal intubation for residents standardized training of anesthesiologists.Methods Forty students,receiving residents standardized training in the department of Anesthesidogy in the Sixth Medical Center of PLA General Hospital were enrolled in the study,and they were randomly divided into the control group(group C,n=20)and the experimental group(group E,n=20).Group E received theoretical education and the training of ORSIM Bronchial simulator,while group C received theoretical education and the training with fiberoptic bronchoscope tracheal intubation model.The theoretical examination scores and the intubation time,the times of hitting the bronchial wall,the sum of rotation angles,the one-time success rate of intubation and the pass rate of operation examination were compared between the two groups.The two groups of students were investigated by questionnaire and their self-confidence was evaluated.Results There was no significant difference in the average scores of multiple choice questions,short answer questions and total scores between the two groups in the theoretical examination(P>0.05).The total intubation time and rotation angle of group E were significantly less than those of group C(P<0.05).The one-time success rate of intubation and the pass rate of operation examination of group E were higher than those of group C(P<0.05).In the questionnaire survey of two groups,the survey results of group E were significantly better than those of group C in the four questions related to enhancing learning interest,increasing teaching affinity,improving practical operation ability and supporting teaching mode(P<0.05);the operation self-confidence level of group E was also significantly higher than that of group C(P<0.05).Conclusion The adoption of ORSIM bronchial simulator makes the learning process more interesting and brings high success rate.Meanwhile,it can improve the residents'initiative research ability,basic clinical skills and self-confidence.It has significant advantages in the teaching of fiberoptic bronchoscope guided endotracheal intubation for standardized training students of anesthesiologists.
