摘要 目的:探讨成人形脊髓纵裂畸形(Split cord malformation,SCM)的影像表现及临床意义。方法:回顾性分析16例经临床证实SCM及影像资料。结果:16例SCM中,男3例,女13例,年龄40~60岁,平均(50.1±5.7)岁。本组病例均发生于腰段。I型14例((87.5%)),II型2例(12.5%)。I型中腹侧完全型7例(50%),腹侧不完全型4例(28.6%),背侧型3例(21.4%)。I型CT及MR轴位图像显示椎管内可见前后走行的间隔将椎管及脊髓一分为二,呈"斗鸡眼征"或"猫头鹰征";Ⅱ型轴位图像显示在同一个硬膜囊内脊髓一分为二,远端可融合或不融合,轴位MR T2WI图像上可表现为"火星人脸"征。结论:SCM的CT及MR表现有其特征形表现,CT结合MR可以明确SCM的分型及骨性间隔的分型,以及了解有无其它伴发病变,为临床治疗方案的选择提供重要依据。 Objective:To explore the adult split cord malformation(SCM) findings and clinical significance.Methods:The SCM and imaging data of 16 cases confirmed clinically were analyzed retrospectively.Results:Among the 16 cases of SCM, there were 3 males and 13 females, aged from 40 to 60 years old, with an average of(50.15.7) years old. All the cases occurred in the lumbar segment. Type I was found in 14 cases(87.5%)), type II in 2 cases(12.5%). Type I included ventral complete type in 7 cases(50%), ventral incomplete type in 4 cases(28.6%), and dorsal type in 3 cases(21.4%). The axial images of type I CT and MR showed that the anterior and posterior interval in the spinal canal divided the spinal canal and spinal cord into two, showing "cross-eye sign" or "owl sign".Type Ⅱ axial images showed that the spinal cord was divided into two parts in the same dural sac, and the distal spinal cord could be fused or not fused, and the "Martian face" sign could be shown on axial MR T2WI images.Conclusion:The CT and MR findings of SCM have their own characteristic features. CT combined with MR can determine the classification of SCM and bony septum, and understand whether there are other concomitant lesions, which provides an important basis for the choice of clinical treatment.
