改良持续皮肤牵张技术治疗大面积皮肤缺损 被引量:5

Modified continuous skin stretch technique to treat large area skin defects

摘要 [目的]介绍改良持续皮肤牵张技术治疗大面积皮肤缺损的手术方法和初步临床疗效。[方法]对7例大面积皮肤缺损患者应用改良持续皮肤牵张技术进行修复治疗。在缺损创面长轴两侧上下创缘分别穿人2枚克氏针,对向牵拉克氏针并于其裸露处以2束双股丝线拉紧并打结,使创面两侧皮缘处于适度紧张状态。每日进行适度牵张,当创面两侧皮缘可对合时,去除装置,对皮肤全层进行水平褥式外翻缝合。[结果]7例患者均顺利闭合创面,未行植皮或皮瓣修复等治疗。伤口愈合时间为18~36d,平均(28.14±6.77)d。所有患者均获创面愈合后半年随访,创缘无皮肤感觉及功能异常。[结论]此改良持续皮肤牵张技术是一种治疗大面积皮肤缺损的有效方法。 [Objective]To introduce the surgical technique and preliminary clinical outcomes of a modified continuous skin stretch technique to treat large area skin defects.[Method]Seven patients with large-area skin defects were treated with the modified continuous skin stretch technique.Two Kirschner wires were inserted into the upper and lower wound edges on both sides of the long axis of the defect respectively,and the Kirschner wires were pulled oppositely,and tightened and knotted with two double strands of silk thread interruptedly,so that the skin edges on both sides of the wound were under moderate tension.Adjust the stretch by tightening the knots every day.Till the edges of the wound on both sides of the wound was closed,remove the device and repair the skin by horizontal mattress suture.[Results]All 7 patients had the wound closed successfully,without skin grafting or flap transfer.The wound healing time was 18 to 36 days,with an average of(28.14±6.77)days.All patients were followed up for 6 months after the wounds healed,and no abnormal skin sensation around the wound,and no function impairment of the limb were noticed in anyone of them.[Conclusion]This modified continuous skin stretch technique is an effective method to treat large areas of skin defects.
