摘要
目的探讨膝关节镜下前交叉韧带断裂自体肌腱重建术对患者膝关节韧带功能和运动功能的影响。方法50例前交叉韧带断裂患者,均采用膝关节镜下前交叉韧带断裂自体肌腱重建术治疗。比较患者手术前后的膝关节角度重现测量值、国际膝关节文献委员会膝关节评估表(IKDC)评分、关节活动度(ROM)、Lysholm膝关节功能评分量表(Lysholm评分)、感觉阈值差距、IKDC分级、体格检查阳性率、KT-1000值。结果健侧膝关节角度重现测量值手术前后比较,差异无统计学意义(P>0.05);患侧膝关节角度重现测量值术后3个月均显著低于术前,差异有统计学意义(P<0.05)。术前,患侧膝关节角度重现测量值均显著高于健侧,差异有统计学意义(P<0.05);术后3个月,健侧与患侧膝关节角度重现测量值比较,差异无统计学意义(P>0.05)。术后12个月,患者的IKDC评分(92.2±3.1)分、ROM(135.2±4.3)°、Lysholm评分(93.1±3.6)分显著高于术前的(45.8±3.8)分、(113.8±12.4)°、(70.2±3.3)分,感觉阈值差距(1.4±0.3)°显著低于术前的(3.4±0.6)°,差异有统计学意义(P<0.05)。术后12个月,患者的IKDC分级优于术前,差异有统计学意义(P<0.05)。术后12个月,患者的Lachman试验阳性率8.0%、前抽屉试验(ADT)阳性率6.0%、KT-1000值(2.5±1.0)mm均低于术前的90.0%、70.0%、(6.3±1.2)mm,差异有统计学意义(P<0.05)。结论膝关节镜下前交叉韧带断裂自体肌腱重建术治疗前交叉韧带断裂能够显著改善膝关节功能,有效恢复韧带功能。
Objective To discuss the effect of arthroscopic anterior cruciate ligament reconstruction with autologous tendon on patients’knee ligament function and motor function.Methods 50 patients with anterior cruciate ligament rapture all underwent arthroscopic anterior cruciate ligament reconstruction with autologous tendon.The reproduction of joint angles,International Knee Documentation Committee(IKDC)score,range of motion(ROM),Lysholm score,sensory thresholds difference,IKDC grading,positive physical examination rate,and KT-1000 values were compared before and after surgery.Results There was no statistically significant difference in reproduction of joint angles on the healthy side before and after surgery(P>0.05).The knee angle to reproduction on the affected side at 3 months after surgery was significantly lower than that before surgery,and the difference was statistically significant(P<0.05).Before surgery,the knee angle to reproduction on the affected side was significantly lower than that on the healthy side,and the difference was statistically significant(P<0.05).At 3 months after surgery,there was no statistically significant difference in knee angle to reproduction between the healthy and affected side(P>0.05).At 12 months after surgery,the IKDC score(92.2±3.1)points,ROM(135.2±4.3)°,Lysholm score(93.1±3.6)points were significantly higher than(45.8±3.8)points,(113.8±12.4)°,(70.2±3.3)points before surgery,and the sensory threshold difference(1.4±0.3)°was significantly lower than(3.4±0.6)°before surgery.The difference was statistically significant(P<0.05).At 12 months after surgery,the IKDC grading of patients was better than that before surgery,and the difference was statistically significant(P<0.05).At 12 months after surgery,the positive rate of Lachman test 8.0%,positive rate of anterior drawer test(ADT)6.0%,and KT-1000 value(2.5±1.0)mm were all lower than 90.0%,70.0%,(6.3±1.2)mm before surgery,and the difference was statistically significant(P<0.05).Conclusion Arthroscopic anterior cruciate ligament reconstruction with autologous tendon can significantly improve the function of knee joint and effectively restore the ligament function.
