摘要
目的观察加味聚精汤联合山海丹颗粒治疗肾虚血瘀型男性不育症的临床疗效。方法选取2020年2月—11月常州市中医医院男科门诊收治的男性不育症辨证属于肾虚血瘀证患者200例,随机分为对照组和观察组,各100例。对照组予口服加味聚精汤汤剂+脉血康胶囊;观察组予口服加味聚精汤汤剂+山海丹颗粒。两组均无间断连续治疗12周后,对比评价临床疗效。观察指标主要为两组治疗前后精液量、精子浓度、前向运动精子率(PR)、精子总活力、精子DNA碎片指数(DFI);治疗期间监测服药安全性。结果(1)200例患者中共166例完成全部治疗及检测,纳入最终的分析,计对照组80例、观察组86例。(2)治疗前两组患者间年龄、精液量、精子浓度、精子活力以及精子形态学指标均无统计学差异(P>0.05);治疗后两组患者的精液量、精子浓度、PR、精子总活力均较治疗前显著改善(P<0.05),且观察组较对照组改善更显著(P<0.05)。两组患者治疗后精子DFI、高可染性精子指数(HDS)均较治疗前显著降低(P<0.05),且治疗后观察组精子DFI显著低于对照组(P<0.05)。(3)治疗后,对照组和观察组分别有3位和6位配偶成功妊娠,观察组临床总有效率显著高于对照组(91.86%vs.88.75%)(P<0.05);治疗过程中两组患者均未观察到明显不良反应。结论加味聚精汤联合山海丹颗粒治疗肾虚血瘀型男性不育症的临床疗效总体优于对照组,尤其在改善精子DFI方面观察组优势明显,值得临床推广应用。
Objective:To observe the clinical effect of Jiawei Jujing decoction combined with Shanhaidan granules in treatment of male infertility with kidney deficiency and blood stasis.Methods:A total of 200 male infertility patients with kidney deficiency and blood stasis syndrome admitted to the andrology clinic of Changzhou Traditional Chinese Medicine Hospital from February 2020 to November 2020 were selected.The patients were randomly divided into control group and observation group with 100 patients in each group.The patients in control group were orally given Jiawei Jujing decoction and Maixuekang capsule.The patients in observation group was orally given Jiawei Jujing decoction and Shanhaidan granules.After continuous treatment for 12 weeks,the clinical efficacy of both groups was compared and evaluated.The main observation indexes were semen volume,sperm concentration,sperm forward motility rate(PR),total sperm motility and sperm DNA fragmentation index(DFI)before and after treatment in the two groups.The safety of medication was monitored during treatment.Results:Among of 200 patients,166 patients including 80 patients in the control group and 86 patients in the observation group completed the treatment and detection.There were no significant differences in age,semen volume,sperm concentration,PR,total sperm motility and sperm morphology before treatment between the two groups(P>0.05).The semen volume,sperm concentration,PR and total sperm motility after treatment were significantly improved compared with before treatment in two groups(P<0.05),and the improvement in observation group was more significant than that in control group(P<0.05).The sperm DFI and high-dye sperm index(HDS)were significantly decreased after treatment in both groups(P<0.05),and sperm DFI after treatment in observation group was significantly lower than control group(P<0.05).Three spouses in the control group and 6 spouses in the observation group were successfully pregnant after treatment.The total clinical effective rate in the observation group was significantly higher than that in the control group(91.86%vs.88.75%)(P<0.05).No obvious adverse reaction was observed during the treatment in both groups.Conclusions:The efficacy of Jiawei Jujing decoction combined with Shanhaidan granules is generally better than the controls in the treatment of male infertility with kidney deficiency and blood stasis,especially in improving sperm DNA fragmentation index,which is worthy of clinical application.
