摘要
目的探讨枯朴洗液对Ⅲ~Ⅳ度混合痔痔上黏膜环形切除钉合(PPH)术后肛周水肿的防治效果。方法选取2019年10月-2020年10月医院收治的124例混合痔患者,采用随机数字表法分为对照组和研究组,各62例,对照组接受PPH术,研究组在PPH术基础上予以枯朴洗液熏洗、坐浴治疗,15 min/次,术前3 d及术后1~7 d连续用药。记录两组手术时间、疼痛消失时间、创面愈合时间及住院时间,评估术后肛周水肿情况、临床疗效,检测手术前后肛肠动力学、血清炎症因子水平,统计治疗期间药物不良反应。结果研究组疼痛消失时间、创面愈合时间及住院时间较对照组短(P<0.05)。术后3、7 d研究组肛周水肿率低于对照组(P<0.05);与术后1 d比较,两组术后3、7 d肛周水肿率均降低,且术后7 d低于术后3 d(P<0.05)。术后7 d,研究组总有效率高于对照组(93.55%vs 80.65%,P<0.05);两组肛管静息压、最大肛管收缩压、肛管高压区长度高于术前,且研究组更高(P<0.05);两组血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、C反应蛋白(CRP)水平低于术前,且研究组更低(P<0.05)。治疗期间两组均无严重不良反应。结论枯朴洗液外用可有效防治Ⅲ~Ⅳ度混合痔PPH术后肛周水肿,改善肛肠动力学指标及炎症反应,安全可靠。
Objective To investigate the preventive and therapeutic effect of Kupu lotion on prolapse and hemorrhoids(PPH)forⅢ~Ⅳdegree mixed hemorrhoids.Methods 124 patients with mixed hemorrhoids admitted to the hospital from October 2019 to October 2020 were selected,and the patients were divided into the control group and the study group by a random number table method,with 62 cases in each group.The control group received PPH operation,and the study group received Kupu lotion fumigation and sitz bath treatment on the basis of PPH operation,15 min per time,3 days before the operation and 1~7 days after the operation.The operation time,pain disappearance time,wound healing time and hospital stay were recorded in the two groups.Postoperative perianal edema and clinical effica-cy were evaluated.Anorectal dynamics indexes and serum inflammatory factor levels were detected before and aftersurgery in the two groups.Adverse drug reactions were counted during the treatment.Results The pain disappearancetime,wound healing time and hospital stay in the study group were shorter than those in the control group(P<0.05).The rate of perianal edema in the study group was lower than that in the control group at 3 and 7 days after the opera-tion,and the rate of perianal edema was lower at 7 days after the operation than 3 days after the operation(P<0.05).Compared with 1 day after the operation,the rate of perianal edema was lower in both of groups on 3 and 7 days afterthe operation(P<0.05).Seven days after the operation,the total clinical effective rate of the study group was higherthan that of the control group 93.55%vs.80.65%(P<0.05).The anal canal resting pressure resting,maximum anal sys-tolic pressure,and the length of the anal high pressure area in the two groups were higher than those before the opera-tion and were higher in the study group(P<0.05).The levels of serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)in the two groups were lower than those before the operation and were lower in thestudy group(P<0.05).There were no serious adverse reactions in the two groups during the treatment.Conclusion Thetopical application of Kupu Lotion can effectively prevent and treat the perianal edema after PPH withⅢ~Ⅳdegreemixed hemorrhoid surgery,and it can improve the anorectal dynamics and inflammation,and it is safe and reliable.
