PD-L1和FOXP3在甲状腺乳头状癌组织中的表达及临床意义 被引量:4

摘要 目的探讨程序性死亡配体1(PD-L1)和叉头翼螺旋转录因子3(FOXP3)在甲状腺乳头状癌(PTC)组织中的表达及与临床病理特征的关系。方法收集2018年1月至2020年2月接受甲状腺切除术患者的组织标本160例,其中PTC 140例,结节性甲状腺肿20例。采用免疫组化染色法检测PTC和结节性甲状腺肿组织中PD-L1和FOXP3的表达情况,分析两者表达与PTC临床病理特征的关系以及两者表达的相关性。结果纳入的140例PTC病例中,86例PD-L1阳性表达,其中低表达37例、中表达26例、高表达16例、极高表达7例,PD-L1表达阳性率为61.4%;84例FOXP3阳性表达,其中低表达37例、中表达19例、高表达8例、极高表达20例,FOXP3的阳性表达率为60.0%。在结节性甲状腺肿组织中,PD-L1和FOXP3表达均为阴性,与两者在PTC组织中的表达水平比较,差异有统计学意义(P=0.000,P=0.001)。PD-L1表达与TNM分期、包膜侵犯、腺外侵犯、远处转移和合并桥本氏甲状腺炎(HT)有关(P<0.05),与年龄、性别、T分期、中央区淋巴结转移、侧方淋巴结转移和肿瘤病灶无关(P>0.05)。FOXP3表达与年龄、包膜侵犯、腺外侵犯和TNM分期有关(P<0.05),而与性别、T分期、中央区淋巴结转移、侧方淋巴结转移、肿瘤病灶、远处转移和合并HT均无关(P>0.05)。在PTC组织中,PD-L1与FOXP3表达呈正相关(r=0.732,P=0.000)。结论PD-L1和FOXP3在PTC组织中表达上调,两者表达与TNM分期和肿瘤局部浸润有关,且两者表达呈正相关。 Objective To study the expression of programmed death ligand 1(PD-L1)and forkhead wing spiral transcription factor 3(FOXP3)in papillary thyroid carcinoma(PTC)tissues,and explore the relationship of their expressions with clinicopathlogical characteristics.Methods From January 2018 to February 2020,160 tissue samples were collected from patients undergoing thyroidectomy,including 140 cases of PTC and 20 cases of nodular goiter.Immunohistochemical staining method was used to detect the expression of PD-L1 and FOXP3 in PTC and nodular goiter tissues.The relationship between the expression of PD-L1 and FOXP3 and the clinicopathological characteristics of patients,as well as the correlation of PD-L1 and FOXP3 expression were analyzed.Results Among the 140 cases of PTC,86 cases were PD-L1 positive,including 37 cases of low expression,26 cases of medium expression,16 cases of high expression and 7 cases of extreme high expression.The positive rate of PD-L1 expression was 61.4%.There were 84 cases of FOXP3 positive expression,including 37 cases of low expression,19 cases of medium expression,8 cases of high expression and 20 cases of extreme high expression.The positive expression rate of FOXP3 was 60.0%.In nodular goiter,the expressions of PD-L1 and FOXP3 were negative.Compared with their expression levels in PTC,the difference was statistically significant(P=0.000,P=0.001).The expression of PD-L1 was related to TNM stage,envelope invasion,extraglandular invasion,distant metastasis and Hashimoto s thyroiditis(HT)(P<0.05),but not to age,sex,T stage,central lymph node metastasis,lateral lymph node metastasis and tumor focus(P>0.05).The expression of FOXP3 was related to age,envelope invasion,extraglandular invasion and TNM stage(P<0.05),but not to gender,T stage,central lymph node metastasis,lateral lymph node metastasis,tumor focus,distant metastasis and HT(P>0.05).There was a positive correlation between PD-L1 and FOXP3 expression in PTC(r=0.732,P=0.000).Conclusion The expression of PD-L1 and FOXP3 is up-regulated in PTC tissues.The positive rate of the two expressions is positively correlated with TNM stage and local tumor invasion.The expression of PD-L1 and FOXP3 in PTC tissues is positively correlated.
