基于中医传承辅助平台(V2.5)分析王孟清教授治疗儿童功能性腹痛用药规律 被引量:2

Analysis on medication rules of professor WANG Mengqing for children with functional abdominal pain based on Traditional Chinese Medicine Inheritance Support System platform(V2.5)

摘要 目的基于中医传承辅助平台(V2.5),分析王孟清教授治疗儿童功能性腹痛(FAP)的用药规律。方法收集2017年1月至2020年6月王教授于湖南中医药大学第一附属医院国医堂门诊治疗的FAP患儿的有效病历,分析其常用药物的使用频数、性味,对高频用药进行聚类分析。结果纳入处方173首,涉及中药125味,使用频数≥30次的药物有20味,其中位于前10位的依次为白芍、甘草、柴胡、枳壳、延胡索、鸡内金、黄连、法半夏、香附和黄芩;药性使用频数最高的为寒性,其次为温性;药味使用频数最高的为苦,其次为辛;常用药物组合为甘草-白芍、白芍-柴胡、甘草-柴胡、甘草-白芍-柴胡等;得到新处方3首。结论王教授治疗儿童FAP的用药具有药味少、处方精的特点,擅于寒温并用,注重顾护脾胃,且常配伍行气药以使补而不滞,涉及主要方剂为四逆散加减,具有疏肝健脾、缓急止痛之功效。 Objective To analyze the medication rules of professor WANG Mengqing for children with functional abdominal pain(FAP)based on Traditional Chinese Medicine Inheritance Support System platform(V2.5).Methods From January 2017 to June 2020,valid medical records of children with FAP treated by professor WANG in outpatient service of TCM hall of The First Affiliated Hospital of Hunan University of Chinese Medicine were collected,and the use frequency as well as property and flavor of commonly used medicinals were analyzed.Cluster analysis was conducted for high-frequency medicinal use.Results Totally 173 prescriptions were included,involving 125 flavors of Chinese materia medica(CMM),and the use frequency of 20 flavors of CMM were more than 30 times,among which the top 10 were Paeoniae radix alba,Glycyrrhizae radix et rhizoma,Bupleuri radix,Aurantii fructus,Corydalis rhizoma,Galli gigerii endothelium corneum,Coptidis rhizoma,Pinelliae rhizoma praeparatum,Cyperi rhizoma and Scutellariae radix.The highest use frequency of medicinal property was cold property medicinals,followed by warm property ones;and the highest use frequency of medicinal flavor was bitter flavour,followed by pungent one.Commonly used medicinal combinations were Glycyrrhizae radix et rhizoma and Paeoniae radix alba,Paeoniae radix alba and Bupleuri radix,Glycyrrhizae radix et rhizoma and Bupleuri radix,and Glycyrrhizae radix et rhizoma,Paeoniae radix alba and Bupleuri radix,etc.Three new prescriptions were obtained.Conclusion Professor WANG’s medications for children with FAP are characterized by less flavors and precise prescriptions.He excels at simultaneous use of cold property and warm property medicinals,pays attention to protecting and reinforcing spleen and stomach,and often combines with regulating qi medicinals to tonify without causing stagnation.The main prescription involved is modified Sini San(四逆散),which has the effects of soothing liver and strengthening spleen as well as relieving spasm and stopping pain.
