摘要
目的:探讨应变式弹性成像和剪切波弹性成像对乳腺良恶性肿瘤的诊断价值。方法:收集2016年1月—2019年1月于我院接受检查的81例(83个病灶)乳腺疾病患者为观察对象,对其分别实施应变式弹性成像(SE)及剪切波弹性成像(SWE)检测,SE检测评估患者弹性评分及应变率比值(SR),SWE检测评估患者平均弹性模量值(Emean)、标准差(SD)及周边正常乳腺组织弹性模量比值(Ratio),以病理检查结果为金标准,比较83个病灶中恶性病变(n=39)与良性病变(n=44)弹性评分、SR、Emean、SD、Ratio,绘制SE及SWE鉴别参数对乳腺良恶性肿瘤病灶工作特征曲线,计算曲线下面积,选出SE及SWE中AUC最大参数,并评估该参数在鉴别不同种类乳腺良恶性肿瘤中的准确性。结果:(1)经病理检查,83个病灶中有39个为恶性肿瘤,44个为良性肿瘤,恶性肿瘤弹性评分、SR、Emean、SD、Ratio均高于良性肿瘤(P<0.05);(2)SE中弹性评分及SR对乳腺良恶性肿瘤鉴别ROC的AUC分别为0.849、0.832,SWE中Emean、SD、Ratio对乳腺良恶性肿瘤鉴别ROC的AUC分别为0.913、0.834、0.798;(3)将弹性评分与Emean对不同种类乳腺肿瘤鉴别准确度进行比较发现,弹性评分对BI-RADS分类中Ⅳa类乳腺良恶性肿瘤的评估准确度低于Emean(P<0.05);弹性评分和Emean在Ⅳc、Ⅴ类、Ⅲ类及Ⅳb类乳腺良恶性肿瘤评估准确度无显著差异(P>0.05)。结论:SE和SWE在乳腺良恶性肿瘤评估中均具有较高的诊断价值,能够较为准确的鉴别良恶性病变,如结合患者BI-RADS分类能够更为精准地选择诊断方式。
Objective:The aim of this study is to compare the differential value of strain elastography and shear-wave elastography in benign and malignant breast tumors.Methods:A total of 81 patients(83 lesions)with breast diseases who were examined in our hospital from January 2016 to January 2019 were selected as the research objects.Strain elastic imagine(SE)and Shear wave elastic imagine(SWE)tests were performed respectively.SE test was used to evaluate elastic score and strain rate ratio(SR)of patients,SWE test was used to evaluate average elastic modulvs(Emean),standard deviation(SD)and Ratio of patients with pathological examination results as the gold standard.Elastic score,SR,Emean,SD and Ratio were compared between malignant lesions(n=39)and benign lesions(n=44)in 83 lesions.The working characteristic curves of SE and SWE identification parameters on benign and malignant breast tumor lesions were drawn and the area under the curves was calculated to select the maximum AUC parameters in SE and SWE.The accuracy of this parameter in differentiating different types of benign and malignant breast tumors was evaluated.Results:(1)Pathological examination showed that 39 out of 83 lesions were malignant and 44 were benign.Elasticity score,SR,Emean,SD and Ratio of malignant tumors were higher than those of benign tumors(P<0.05);(2)The AUC of elastic score and SR in SE for differentiating ROC from benign and malignant breast tumors were 0.849 and 0.832,respectively;the AUC of Emean,SD and Ratio in SWE for differentiating ROC from benign and malignant breast tumors were 0.913,0.834 and 0.798;(3)By comparing the accuracy of elastic score and Emean in identifying different types of breast tumors,it was found that the accuracy of elastic score in evaluating Ⅳa benign and malignant breast tumors in BI-RADS classification was lower than Emean(P<0.05),but elasticity scoring and Emean of Ⅳc、Ⅴ、Ⅲ and Ⅳb benign and malignant breast tumors assessment between the two groups(P>0.05).Conclusion:SE and SWE have high diagnostic value in the evaluation of benign and malignant breast tumors,which can accurately distinguish benign and malignant lesions and can more accurately select the diagnosis mode with the combination of bi-RADS classification of patients.
