摘要
目的探究促甲状腺激素(TSH)抑制疗法对老年甲状腺癌患者甲状腺全切除术后的疗效分析及对血清甲状腺球蛋白(Tg)、TSH、癌胚抗原(CEA)水平的影响。方法选取老年甲状腺癌甲状腺全切除术后患者76例,根据是否进行TSH抑制疗法分为观察组(40例)和对照组(36例),对照组采用TSH替代疗法,观察组采用TSH抑制疗法,治疗6个月,观察两组临床疗效、甲状腺功能[TSH、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)]、肿瘤标志物(Tg、CEA)、T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))。结果观察组总有效率高于对照组(90.00%vs 72.22%)(P<0.05)。观察组TSH水平低于对照组,FT3、FT4水平高于对照组(P<0.05)。观察组Tg和CEA水平低于对照组(P<0.05)。观察组CD3^(+)和CD4^(+)水平高于对照组,CD8^(+)水平低于对照组(P<0.05)。结论老年甲状腺癌患者甲状腺全切除术后采用TSH抑制疗法,对甲状腺功能影响小,可降低TSH水平且能抑制Tg、CEA表达,改善免疫功能,进而临床疗效较好。
Objective To explore the effect of thyroid-stimulating hormone(TSH)suppression therapy in elderly patients with thyroid cancer after total thyroidectomy and on serum thyroglobulin(Tg),TSH,and carcinoembryonic antigen(CEA)levels.Methods 76 elderly patients with thyroid cancer via total thyroidectomy were selected and divided into the observation group(40 cases)and the control group(36 cases)according to whether TSH suppression therapy was performed.The control group was treated with TSH replacement therapy,and the observation group was treated with TSH suppression therapy.After 6 months,the clinical efficacy,thyroid function[TSH,free triiodothyronine(FT3),free thyroxine(FT4)],tumors markers(Tg,CEA),T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+))were observed in the 2 groups.Results The total effective rate of the observation group was higher than that of the control group(90.00%vs 72.22%)(P<0.05).The TSH level of the observation group was lower than that of the control group,and the levels of FT3 and FT4 were higher than those of the control group(P<0.05).The levels of Tg and CEA in the observation group were lower than those in the control group(P<0.05).The CD3^(+)and CD4^(+)levels of the observation group were higher than those of the control group,and the CD8^(+)level was lower than that of the control group(P<0.05).Conclusion The TSH suppression therapy in elderly patients with thyroid cancer after total thyroidectomy has little effect on thyroid function,and it can reduce the level of TSH and inhibit the expression of Tg and CEA,and improve immune function with better clinical effect.
