太极拳功法联合情景模式技术对脑卒中平衡功能障碍患者肢体功能恢复的影响分析 被引量:1

摘要 目的分析太极拳功法联合情景模式技术对脑卒中平衡功能障碍患者肢体功能恢复的影响。方法选取2019年3月至2021年3月上海市第二康复医院收治的120例脑卒中平衡功能障碍患者为研究对象。使用随机数字表法将入选患者分别纳入A组、B组、C组、D组,每组各30例,均予常规康复治疗,B组患者联合情景模式技术治疗,C组患者联合太极拳功法治疗,D组患者联合情景模式技术+太极拳功法治疗。比较四组患者治疗前、治疗后1周、治疗后3周、治疗后6周简易平衡评定系统测试(mini-balance evaluation systems test,mini-BESTest)、Berg平衡量表(Berg balance scale,BBS)、下肢Fugl-Meyer量表(Fugl-Meyer assessment,FMA)、功能性步行能力(functional ambulation category,FAC)量表评分变化,并比较各组患者肢体功能恢复情况。结果B组、C组、D组患者治疗后3周mini-BESTest评分均显著高于本组治疗前(均P<0.05)。四组患者治疗后6周mini-BESTest评分均显著高于本组治疗前(均P<0.05)。四组患者治疗后1周、3周和6周BBS评分均显著高于本组治疗前(均P<0.05)。B组、C组、D组患者治疗后1周、3周和6周下肢FMA评分均显著高于本组治疗前(均P<0.05)。四组患者治疗后3周、6周下肢FMA评分均显著高于本组治疗前(均P<0.05)。B组、C组患者治疗后1周、3周、6周BBS评分、下肢FMA评分、FAC评分均显著高于同期A组(均P<0.05)。D组患者治疗后1周mini-BESTest评分显著高于A组(P<0.05),治疗后3周和6周mini-BESTest评分均显著高于同期A组、B组、C组(均P<0.05)。D组患者治疗后1周、3周、6周BBS评分、下肢FMA评分、FAC评分均显著高于同期A组、B组、C组(均P<0.05)。结论太极拳功法和情景模式技术均有助于改善脑卒中平衡功能障碍患者动态平衡功能、下肢功能及步行功能,二者联合能够进一步缩短患者康复时间、提高康复质量。 Objective To analyze the effect of taijiquan exercise combined with situation pattern technique on limb function recovery of stroke patients with balance dysfunction.Method A total of 120 stroke patients with balance dysfunction who were admitted to the Second Rehabilitation Hospital of Shanghai from March 2019 to March 2021 were included in the prospective controlled study.Patients were divided into group A,group B,group C and group D,with 30 cases in each group,using random number table method.All patients were given conventional rehabilitation therapy.Group B patients were treated with combination of situation pattern technique,group C patients were treated with taijiquan exercise therapy,and group D patients were treated with situation pattern technique combined with taijiquan exercise therapy.The scores of the mini-balance evaluation systems test(mini-BESTest),Berg balance scale(BBS),lower limb Fugl-Meyer assessment(FMA),and functional ambulation category(FAC)were compared among the four groups before treatment,1 week after treatment,3 weeks after treatment and 6 weeks after treatment.Result The mini-BESTest scores of patients in group B,group C and group D at 3 weeks after treatment were significantly higher than those before treatment(all P<0.05).The mini-BESTest scores of four groups at 6 weeks after treatment were significantly higher than those before treatment(all P<0.05).The BBS scores of patients in the four groups were significantly higher at 1 week,3 weeks and 6 weeks after treatment than before treatment(all P<0.05).The FMA scores of lower limbs of patients in group B,group C and group D at 1 week,3 weeks and 6 weeks after treatment were significantly higher than those before treatment(all P<0.05).The FMA scores of lower limbs in the four groups at 3 weeks and 6 weeks after treatment were significantly higher than those before treatment(all P<0.05).The BBS score,FMA score of lower limbs and FAC score of patients in group B and group C were significantly higher than those in group A at 1 week,3 weeks and 6 weeks after treatment(all P<0.05).The mini-BESTest scores of patients in group D were significantly higher than those in group A at 1 week after treatment(P<0.05),and the mini-BESTest scores of patients in group D were significantly higher than those in group A,group B and group C at 3 weeks and 6 weeks after treatment(all P<0.05).The BBS score,FMA score of lower limbs and FAC score in group D were significantly higher than those in group A,group B and group C at 1 week,3 weeks and 6 weeks(all P<0.05).Conclusion Both taijiquan exercise combined with and situation pattern technique are helpful to improve dynamic balance function,low limb function and walking function of stroke patient with balance dysfunction,and that combination of the two techniques can further shorten the rehabilitation time and improve the rehabilitation quality.
