摘要
目的探讨区域性医疗质量评价体系对开发区类地区区域医疗质量的评价效果。方法以成都高新区为例,抽取区域内基层卫生服务机构(11家)、二级及以上综合医院(9家)和专科医院(8家)共计28家机构,采用自行开发和设计的《区域医疗质量评价指标体系》对三类机构进行评价,并进行组间比较。结果基层医疗机构在医疗安全与行风建设、处方点评方面得分高于综合医院和专科医院(P<0.05),但综合医院和专科医院的医疗安全与行风建设、处方点评得分比较,差异无统计学意义(P>0.05)。无住院业务的机构多项评分高于有住院业务的机构,但护理管理和检验管理得分较低,其中护理管理方面差异有统计学意义(P<0.001)。结论自主开发的区域医疗质量评价指标体系能较为客观、准确地获取了辖区内多类机构的医疗质量水平,验证了评价体系的有效性,发现了住院业务对医疗质量的影响,为区域医疗质量的进一步提高以及类似地区医疗卫生事业发展提供了研究证据。
Objective To explore the effect of regional medical quality evaluation system on the evaluation of regional medical quality in development zones. Methods Chengdu High-tech Zone was taken as an example. A total of 28 institutions in the region, including primary health service institutions(n=11), second-class and above general hospitals(n=9), and specialized hospitals(n=8), were selected. The self-developed and designed Regional Medical Quality Evaluation Index System was used to evaluate the three types of institutions. And the comparison among groups was made.Results In medical safety, standardized management and prescription review, primary medical institutions scored higher than general hospitals and specialized hospitals(P<0.05), but there was no significant difference between general hospitals and specialized hospitals(P>0.05). Institutions without inpatient service scored higher than those with inpatient service in most indexes, but they scored lower in nursing management and clinical laboratory management, and the difference in nursing management was statistically significant(P<0.001).Conclusion The self-developed Regional Medical Quality Evaluation Index System can objectively and accurately evaluate the medical quality level of multiple types of institutions within the jurisdiction, which verified the effectiveness of the evaluation system, found the influence of inpatient service on medical quality, and provides research evidences for the further improvement of regional medical quality and the development of medical and health services in similar development zones.
