摘要
目的探讨阴道前后壁联合修复在产后中重度阴道松弛矫正中的应用。方法选取我院2020年1月-2021年6月收治的22例中重度阴道松弛伴有阴道前壁轻度脱垂或者压力性尿失禁患者,采用阴道前壁黏膜纵行切开、剥离,黏膜下缝合加强阴道前壁结构;阴道后壁及侧壁U型切开、剥离,缝合肛提肌、球海绵体肌,重建会阴体,塑形阴道前庭。结果22例患者切口均一期愈合,未出现感染、直肠、膀胱等邻近器官损伤,术后随访3个月到半年,性生活满意度显著提高,8例伴有压力性尿失禁患者症状明显改善。结论阴道前后壁联合修复不但修复阴道前壁脱垂、减轻了压力性尿失禁的症状;而且通过阴道后壁黏膜下肛提肌、球海绵体肌的缝合固定,达到了加强了盆地支撑结构、重建会阴体、恢复阴道正常弹性及握持力的效果。
Objective To explore the application of combined repair of anterior and posterior vaginal wall in the correction of moderate and severe postpartum vaginal relaxation.Methods From January 2020 to June 2021,22 patients with moderate and severe vaginal relaxation with mild prolapse of the anterior vaginal wall or stress urinary incontinence were selected.The anterior vaginal wall mucosa was cut and stripped longitudinally,and submucosal suture was used to strengthen the anterior vaginal wall structure;U-shaped incision and stripping of the posterior and lateral walls of the vagina,suture the levator ani muscle and bulbocavernous muscle,reconstruct the perineum and shape the vaginal vestibule.Results The incisions of 22 patients healed in one stage without infection,rectum,bladder and other adjacent organ damage.After follow-up for 3 months to half a year,the sexual life satisfaction was significantly improved,and the symptoms of 8 patients with stress urinary incontinence were significantly improved.Conclusion The combined repair of anterior and posterior vaginal wall can not only repair the prolapse of anterior vaginal wall and reduce the symptoms of stress urinary incontinence;Moreover,through the suture and fixation of submucosal levator ani muscle and bulbocavernous muscle on the posterior wall of the vagina,the effect of strengthening the basin support structure,reconstructing the perineum,and restoring the normal elasticity and grip of the vagina is achieved.
