摘要
目的:探讨不同临床特征乳腺癌(BC)患者血清缺氧诱导因子-1α(HIF-1α)、25-羟维生素D_(3)[25(OH)D_(3)]、胰岛素样生长因子-1(IGF-1)及血清淀粉样蛋白A(SAA)水平表达情况及其临床意义。方法:收集2017年3月-2020年7月深圳市龙华区中心医院收治的92例乳腺癌患者作为BC组,113例乳腺良性肿瘤(BBT)患者作为BBT组,120名健康育龄妇女作为对照组。分别检测血清中HIF-1α、25(OH)D_(3)、IGF-1及SAA水平。结果:BC组HIF-1α、IGF-1及SAA水平明显高于BBT组和对照组,而25(OH)D_(3)水平明显低于BBT组和对照组,差异均有统计学意义(P<0.05);BBT组IGF-1和SAA水平均高于对照组,差异均有统计学意义(P<0.05),而HIF-1α、25(OH)D_(3)与对照组比较,差异均无统计学意义(P>0.05)。Ⅲ、Ⅳ期、肿块>5.0 cm、有淋巴结转移及组织中、低度分化的BC患者血清HIF-1α、IGF-1及SAA水平分别明显高于Ⅰ、Ⅱ期、肿块<2.0 cm、无淋巴结转移及组织高度分化的BC患者,而25(OH)D_(3)水平明显低于Ⅰ、Ⅱ期、肿块<2.0 cm、无淋巴结转移及组织高度分化的BC患者,差异均有统计学意义(P<0.05)。经Pearson相关性分析,BC患者血清HIF-1α与IGF-1、HIF-1α与SAA、IGF-1与SAA均呈正相关(r=0.587、0.752、0.604,P<0.05),而25(OH)D_(3)与HIF-1α、IGF-1、SAA均呈负相关(r=-0.494、-0.715、-0.570,P<0.05)。结论:BC患者HIF-1α、IGF-1及SAA水平明显升高,而25(OH)D_(3)水平明显降低,且与不同临床特征有关。因此,加强上述指标监测对BC患者早期鉴别诊断及疾病严重程度的判断具有一定的指导意义。
Objective:To investigate the levels of hypoxia inducible factor-1α(HIF-1α),25-hydroxyvitamin D_(3)[25(OH)D_(3)],insulin-like growth factor-1(IGF-1)and serum amyloid A(SAA)in breast cancer(BC)patients with different clinical features and their clinical significance.Method:From March 2017 to July 2020,92 breast cancer patients in Shenzhen Longhua District Central Hospital were enrolled as BC group,113 benign breast tumor(BBT)patients were enrolled as BBT group,and 120 healthy women of childbearing age were enrolled as control group.The levels of HIF-1α,25(OH)D_(3),IGF-1 and SAA were detected.Result:The levels of HIF-1α,IGF-1 and SAA in BC group were significantly higher than those in BBT group and control group,while the level of 25(OH)D_(3)was significantly lower than those in BBT group and control group,the differences were statistically significant(P<0.05).The levels of IGF-1 and SAA in BBT group were higher than those in control group,and the differences were statistically significant(P<0.05),while there were no significant differences in HIF-1αand 25(OH)D_(3)(P>0.05).The levels of HIF-1α,IGF-1 and SAA in BC patients with stageⅢandⅣ,mass>5.0 cm,lymph node metastasis and medium and low differentiation were significantly higher than those in BC patients with stageⅠandⅡ,mass<2.0 cm,no lymph node metastasis and high differentiation,while the level of 25(OH)D_(3)was significantly lower than those in BC patients with stageⅠandⅡ,mass<2.0 cm,no lymph node metastasis and high differentiation,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that HIF-1αand IGF-1,HIF-1αand SAA,IGF-1 and SAA were positively correlated(r=0.587,0.752,0.604,P<0.05),while there was a negative correlation between 25(OH)D_(3)and HIF-1α,IGF-1,SAA(r=-0.494,-0.715,-0.570,P<0.05)in BC patients.Conclusion:The levels of HIF-1α,IGF-1 and SAA are significantly increased in BC patients,while the level of 25(OH)D_(3)is significantly decreased,which are related to different clinical characteristics.Therefore,strengthening the monitoring of the above indicators has certain guiding significance for the early and differential diagnosis of BC patients and the judgment of the severity of the disease.
