摘要
目的探究七味三芎汤联合低分子肝素治疗股骨粗隆间骨折术后下肢深静脉血栓(Deep Vein Thrombosis,DVT)的临床疗效。方法选择2017年1月—2019年12月于樟树市中医医院行股骨粗隆间骨折手术的患者70例,按随机数字表法分为2组,各35例。对照组给予低分子肝素治疗,研究组在对照组基础上联合七味三芎汤治疗。2组治疗前后各项凝血功能指标水平、血液流变学指标及DVT发生率比较。结果相比于对照组,研究组活化部分凝血酶时间(Activated partial thromboplastin time,APTT)、凝血酶原时间(Prothrombin time,PT)水平较高,D二聚体(D-Dimer,D-D)水平、血液流变学各指标水平均较低,差异有统计学意义(P<0.05);研究组DVT发生率低于对照组,差异有统计学意义(P<0.05)。结论七味三芎汤联合低分子肝素可有效预防股骨粗隆间骨折术后DVT的形成,降低血液凝固性,改善机体微循环,有利于术后恢复。
Objective To explore the clinical efficacy of Qiwei Sanxiong decoction combined with low molecular weight heparin in the treatment of deep vein thrombosis(DVT)of lower limbs after femoral intertrochanteric fracture.Methods Seventy patients who underwent femoral intertrochanteric fracture surgery in Zhangshu Hospital of Traditional Chinese Medicine from January 2017 to December 2019 were selected and divided into two groups according to the random number table method,with 35 cases in each group.The control group was treated with low molecular weight heparin,and the study group was treated with Qiwei Sanxiong decoction on the basis of the control group.The levels of coagulation function indexes,hemorheology indexes and the incidence of DVT were compared between the two groups before and after treatment.Results Compared with the control group,the study group has higher levels of activated partial thrombin time(APTT)and prothrombin time(PT),and lower levels of D-dimer(DD)and hemorheology indicators.There was statistical significance(P<0.05);the incidence of DVT in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Qiwei Sanxiong decoction combined with low molecular weight heparin can effectively prevent the formation of DVT after femoral intertrochanter fracture,reduce blood coagulation,improve the microcirculation of the body,and be beneficial to postoperative recovery.
