受控衰减参数对初发2型糖尿病患者胰岛素抵抗评估的应用价值 被引量:1

Application value of controlled attenuation parameters in evaluating insulin resistance in newly diagnosed patients with type 2 diabetes mellitus

摘要 目的探讨受控衰减参数(CAP)对初发2型糖尿病(T2DM)患者胰岛素抵抗(IR)的评估价值。方法采用回顾性分析方法,选取2019年6月至2020年12月在华南理工大学附属第六医院内分泌科确诊初发T2DM患者为DM组(n=60),选取同期与初发T2DM患者年龄、性别相匹配的健康人群为非DM组(n=60)。采用Spearman相关分析了解CAP与IR之间的相关性,采用ROC曲线分析CAP指数诊断初发T2DM患者IR的效能。结果DM组患者体重指数(BMI)、内脏脂肪面积(VFA)、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、超敏C反应蛋白(hs-CRP)和CAP明显高于非DM组,高密度脂蛋白胆固醇(HDL-C)明显低于非DM组,差异均有统计学意义(P<0.05);初发T2DM患者的HOMA-IR与VFA、TC、TG、LDL-C、FINS、FPG、HbA1c、hs-CRP呈正相关(P<0.05),与HDL-C呈负相关(P<0.05);初发T2DM患者CAP与HOMA-IR、VFA、TC、TG、LDL-C、FINS、FPG、HbA1c、hs-CRP呈正相关(P<0.05),与HDL-C呈负相关(P<0.05);CAP评估初发T2DM患者IR的曲线下面积AUC(95%CI)为0.806(0.693~0.919)。结论瞬时弹性成像指标CAP与初发T2DM患者IR密切相关,能够有效评估初发T2DM患者IR。 Objective To investigate the value of controlled attenuation parameters(CAP)in evaluating insulin resistance(IR)in newly diagnosed patients with type 2 diabetes mellitus(T2DM).Methods The clinical data of newly diagnosed patients with T2DM admitted to the Sixth Affiliated Hospital of South China University of Technology from June 2019 to December 2020 were analyzed retrospectively,and they were set as the DM group(n=60).In the same period,healthy people matched with the age and gender of newly diagnosed patients with T2DM were selected as the non-DM group(n=60).Spearman correlation analysis was used to get the correlation between CAP and IR,and ROC curve was used to analyze the efficacy of CAP index in diagnosing IR in newly diagnosed patients with T2DM.Results In the DM group,BMI,VFA,FPG,HbA1c,FINS,HOMA-IR,TC,TG,LDL-C,hs-CRP and CAP were significantly higher than those in the non-DM group,while HDL-C was significantly lower than that in the non-DM group,with statistically significant differences in all groups(P<0.05).HOMA-IR was positively correlated with VFA,TC,TG,LDL-C,FINS,FPG,HbA1c and hs-CRP(P<0.05),but negatively correlated with HDL-C(P<0.05).In newly diagnosed patients with T2DM,CAP was positively correlated with HOMA-IR,VFA,TC,TG,LDL-C,FINS,FPG,HbA1c and hs-CRP(P<0.05),but negatively correlated with HDL-C(P<0.05).The area under the curve AUC(95%CI)of CAP for evaluating IR of newly diagnosed patients with T2DM was 0.806(0.693~0.919).Conclusion Transient elastography index CAP is closely related to IR of newly diagnosed patients with T2DM and can effectively evaluate IR of newly diagnosed patients with T2 DM.
