摘要
目的比较膝关节镜辅助下内固定和传统切开内固定治疗后外侧胫骨平台骨折的效果。方法抽取2019年7月至2020年9月郑州市第一人民医院收治的后外侧胫骨平台骨折患者102例, 按照手术方案分为两组。对照组51例给予传统切开内固定术, 观察组51例给予膝关节镜辅助下内固定术。比较两组手术效果、围术期指标、相关评分及并发症。结果观察组优良率为92.16%(47/51), 高于对照组的76.47%(39/51), χ^(2)=4.744, P=0.029。观察组手术时间、切口长度、切口愈合时间少于对照组(t=14.528、21.385、11.198, P<0.05)。术后5 d观察组疼痛评分低于对照组(t=32.266, P<0.05), 术后6个月观察组主观评分高于对照组(t=9.073, P<0.05)。观察组并发症发生率为5.88%(3/51), 少于对照组的11.76%(6/51), χ^(2)=4.744, P=0.029。结论与传统切开内固定比较, 膝关节镜辅助下内固定治疗后外侧胫骨平台骨折优良率更高, 可改善围术期指标, 缓解术后疼痛, 且并发症发生率低。
Objective To compare the comparison of effects of knee arthroscopy-assisted internal fixation and traditional incision and internal fixation on posterolateral tibial plateau fractures.Methods A total of 102 patients with posterolateral tibial plateau fractures admitted to Zhengzhou First People’s Hospital from July 2019 to September 2020 were selected and divided into two groups according to the surgical plan.Fifty-one patients in the control group were treated with traditional incision and internal fixation,and 51 patients in the observation group were treated with knee arthroscopy-assisted internal fixation.The surgical effects,perioperative indicators,related scores and complications were compared between the two groups.Results The excellent and good rate in the observation group was 92.16%(47/51),which was higher than the 76.47%(39/51)in the control group,χ^(2)=4.744,P=0.029.The operation time,incision length,and incision healing time in the observation group were shorter than those in the control group(t=14.528,21.385,11.198,P<0.05).The pain score in the observation group was lower than that in the control group at 5 days after operation(t=32.266,P<0.05).The subjective score in the observation group was higher than that in the control group at 6 months after operation(t=9.073,P<0.05).The complication rate in the observation group was 5.88%(3/51),which was less than 11.76%(6/51)in the control group,χ^(2)=4.744,P=0.029.Conclusions Compared with traditional open internal fixation,knee arthroscopy-assisted internal fixation on posterolateral tibial plateau fractures has a higher rate of excellence,can improve perioperative indicators,relieve postoperative pain,and have a lower incidence of complications.
