摘要
目的系统评价在运动康复治疗基础上,应用炙甘草汤辅助治疗慢性心力衰竭的临床疗效。方法基于慢性心力衰竭、运动康复治疗、炙甘草汤等相关主题,检索知网(CNKI)、万方数据库、PubMed、维普、龙源等数据库文献资源,下载整理并进行筛选,最终纳入文献7篇,文献中对照组采用常规西药方案结合运动康复治疗,观察组在对照组基础上,给予炙甘草汤治疗,1剂/d,并从中整理归纳出总有效率、证候积分、6 min步行距离、左室射血分数LVEF%共4项指标,进行系统性分析。结果观察组总有效率明显高于对照组(OR=5.49,95%CI:3.16~9.51,Z=6.06,P<0.000 01);相较于对照组,观察组证候积分明显更低,差异具有统计学意义(MD=-4.99,95%CI:-8.33~-1.65,Z=2.93,P=0.003);观察组6 min步行距离明显要更高,差异具有统计学意义(MD=40.39,95%CI:7.86~72.92,Z=2.43,P=0.01);观察组左心室射血分数LVEF%要明显更高,差异具有统计学意义(MD=4.55,95%CI:3.48~5.61,Z=8.38,P<0.000 01)。结论在常规西药方案结合运动康复治疗基础上,应用炙甘草汤辅助治疗慢性心力衰竭治疗疗效显著,可有效缓解临床症状,使受损心肌细胞得到修复,且可有效改善机体心功能。
Objective To systematically evaluate the clinical efficacy of Zhigancao Decoctionin(炙甘草汤)the treatment of chronic heart failure based on sports rehabilitation.Methods Based on chronic heart failure,exercise rehabilitation,Zhigancao Decoction and other related topics,we searched CNKI,Wanfang database,PubMed,Weipu,Longyuan and other database literature resources,downloaded and sorted and screened,and finally included 7 documents.In the literature,the control group was treated with conventional western medicine combined with exercise rehabilitation,and the observation group was treated with Zhigancao Decoction on the basis of the control group,1 dose/d.The total effective rate,syndrome score,6 min walking distance andfour indicators of left ventricular ejection fraction(LVEF%)were systematically analyzed.Results The total effective rate of the observation group was significantly higher than that of the control group(OR=5.49,95%CI:3.16-9.51,Z=6.06,P<0.00001).Compared with that of the control group,the observation group’s syndrome score was significantly lower and the difference was statistically significant(MD=-4.99,95%CI:-8.33--1.65,Z=2.93,P=0.003).The 6-min walking distance in the observation group was significantly higher,and the difference was statistically significant(MD=40.39,95%CI:7.86-72.92,Z=2.43,P=0.01).The left ventricular ejection fraction(LVEF%)in the observation group was significantly higher,and the difference was statistically significant(MD=4.55,95%CI:3.48-5.61,Z=8.38,P<0.00001).Conclusion On the basis of conventional western medicine and exercise rehabilitation therapy,the application of Zhigancao Decoction in the treatment of chronic heart failure has significant curative effect,which can effectively relieve clinical symptoms,repair damaged cardiomyocytes,and effectively improve the body’s heart function.
