摘要
目的观察益气化瘀醒脑汤对脑卒中急性期抑郁患者的治疗效果及对神经递质失衡的调节机制研究。方法将2018年1月—2020年2月医院神经内科收治的符合急性期脑卒中并发抑郁诊断标准的患者82例分为观察组和对照组,每组41例。两组治疗期间均给予卒中后基础治疗,对照组采用氟哌噻吨美利曲辛片治疗,1片/d。观察组采用氟哌噻吨美利曲辛片+益气化瘀醒脑汤治疗。两组各治疗12周。比较两组治疗疗效,HAMD评分、PSQI评分和NIHSS评分,神经递质水平变化。结果观察组治疗总有效率高于对照组(P<0.05);观察组治疗后中医证候积分低于对照组(P<0.05);两组治疗后HAMD评分、PSQI评分和NIHSS评分较治疗前降低,观察组治疗后HAMD评分、PSQI评分和NIHSS评分低于对照组(P<0.05);两组治疗后神经递质5-HT、DA水平较治疗前升高,观察组治疗后神经递质5-HT、DA水平高于对照组(P<0.05)。结论益气化瘀醒脑汤结合西医常规更有利于脑卒中急性期抑郁症患者中医证候的改善和神经功能的恢复,提高患者血浆5-HT、DA水平,降低患者的抑郁程度,提高患者的睡眠质量,疗效优于单纯西药治疗。
Objective To observe the therapeutic effect of Yiqi Huayu Xingnao Decoction(益气化瘀醒脑汤)on depression of patients with acute stroke and the regulation mechanism of neurotransmitter imbalance.Methods Eighty-two patients with acute stroke complicated with depression admitted to our hospital from January 2018 to February 2020 were selected and divided into observation group and control group according to randomized control method,41 patients in each group.During the treatment period,both groups were given basic post-stroke treatment,while the control group was treated with Flurperthiazide Melitrazine Tablets(1 tablet/d)at the same time of basic treatment.The observation group was treated by Yiqi Huayu Xingnao Decoction orally on the basis of the control group.Four weeks was one course of treatment,and both groups received three courses of treatment.The therapeutic effect,SYNDROME score,HAMD score,PSQI score,NIHSS score and neurotransmitter level of the two groups were compared.Results The total effective rate in the observation group was higher than that in the control group(P<0.05).The scores of TCM syndromes in the observation group were lower than those in the control group(P<0.05).HAMD score,PSQI score and NIHSS score after treatment were lower in the two groups than those before treatment,while HAMD score,PSQI score and NIHSS score after treatment in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of 5-HT and DA of neurotransmitters in the two groups were increased compared with those before treatment,while the levels of 5-HT and DA in the observation group were higher than those in the control group(P<0.05).Conclusion Yiqi Huayu Xingnao Decoction combined with conventional western medicine is more beneficial to the improvement of TCM symptoms and the recovery of neurological function in patients with acute depression after stroke.It can increase the levels of 5-HT and DA in serum,reduce the degree of depression,and improve the sleep state of patients.The curative effect is better than that of western medicine alone.
