血清血红素加氧酶-1水平与脓毒症继发急性呼吸窘迫综合征相关性研究 被引量:4

The Correlation Between Serum Heme Oxygenase-1 and Acute Respiratory Distress Syndrome in Sepsis Patients

摘要 目的探讨氧化应激过程中的关键酶血红素加氧酶-1(heme oxygenase-1,HO-1)与脓毒症继发急性呼吸窘迫综合征相关性。方法收集外科、急诊及呼吸重症监护室的脓毒症患者,并按照急性呼吸窘迫综合征标准分为ARDS组与非ARDS组,记录各组患者年龄,性别及入ICU第一个24 h内各项实验室指标,并留取静脉血处理后的血清样本。利用ELISA法测定HO-1的水平。结果本研究共纳入83例脓毒症患者,其中ARDS组34例,非ARDS组49例。结果显示血清HO-1在脓毒症继发ARDS组低表达,且差异有统计学意义(P<0.001).另外,死亡组中血清HO-1水平较生存组显著降低,P<0.05。HO-1水平同其他脏器损伤相关,如弥散性血管内凝血。结论血清HO-1同脓毒症继发ARDS相关,且其同脓毒症预后及DIC的发生同样相关。 Objective ARDS of sepsis patients is one of the key causes of death.Previous studies had proved that heme oxygenase-1(HO-1),a key enzyme of oxidative stress,is related to acute lung injury in rat,but there was still no evidence from patients.Methods Sepsis patients were recruited from Surgical,Emergency and Respiratory Intensive Care Units of our hospital.Those patients were divided into the ARDS group and non-ARDS group.Age,gender and lab results in first 24 hours were all recorded and serum samples were also retained.Levels of serum HO-1 were detected by ELISA.Results A total of 83 sepsis patients were recruited in our study,including 34 ARDS patients and 49 non-ARDS patients.Results showed that levels of serum HO-1 were significantly lower in ARDS than in non-ARDS patients(P<0.001).In addition,serum HO-1 concentration in the death group was significantly lower than in the survival group(P<0.05).HO-1 levels were also significantly lower in sepsis patients with diffuse intravascular coagulation.Conclusion Serum HO-1 is corelated with ARDS of sepsis patients,and lower levels of HO-1 are also related to a poor prognosis of those patients.
