摘要
目的研究老年吞咽障碍(DD)患者营养支持需求情况,并分析影响患者营养支持需求可能的因素。方法前瞻选取2018年1月至2019年2月首都医科大学附属北京友谊医院老年医学科接收180例DD患者为研究对象,患者入院时参照微型营养评定法(MNA)评估营养状况,以评价营养支持需求情况;设计一般资料调查问卷,记录患者基线资料,经单因素与多因素检验,分析老年DD患者营养支持需求的影响因素。结果180例老年DD患者经评估,患者营养支持需求较高86例(47.78%),需求一般94例(52.22%);初步比较不同营养支持需求患者的资料后,经Logistic回归分析结果显示,合并慢性疾病、咀嚼功能低下、家庭月收入低均、日常生活能力低下均是老年DD患者营养支持需求高的影响因素(OR>1,P<0.05)。结论老年DD患者营养支持需求普遍偏高,可能受患者合并慢性疾病、咀嚼功能低下、家庭月收入低、日常生活能力低下等因素影响,临床应重点关注上述因素,重视早期干预。
Objective To investigate the nutritional support needs in elderly patients with deglutition disorders(DD)and its influencing factors.Methods A total of 180 elderly patients with DD who were treated in our hospital were enrolled in the study.The nutritional status of patients on admission was evaluated by mini nutritional assessment(MNA)to assess the nutritional support needs,and general data questionnaire was used to record the baseline data of patients.Moreover the influencing factors of nutritional support needs were analyzed by single factor and multi factor tests.Results Among the 180 patients,86 patients had high demand for nutritional support,accounting for 47.78%,and 94 patients had general demand,accounting for 52.22%.After preliminary comparing the data of patients with different nutritional support needs,Logistic regression analysis showed that combining with chronic diseases,low masticatory function,low family monthly income and low activities of daily living were the influencing factors of high nutritional support needs in elderly patients with DD(OR>1,P<0.05).Conclusion The nutritional support needs of elderly patients with DD are generally high,which may be affected by combining with chronic diseases,low masticatory function,low family monthly income and low activities of daily living,and the above factors should be paid much attention to in clinical practice,it is necessary to carry out early intervention.
