摘要
目的探讨急性心肌梗死(AMI)病人血清磷脂酶A2(PLA2)和同型半胱氨酸(Hcy)水平与心室重构的关系。方法选择2015年3月—2018年6月在河南省开封市中心医院就诊的AMI病人100例,并根据是否左心室重构分为重构组(38例)、非重构组(62例),另选取同期体检的健康者50名作为对照组。检测血清PLA2、Hcy、Ⅰ型N端胶原前肽(PⅠNP)、Ⅰ型C端胶原末肽(CⅠTP)、转化生长因子-β_(1)(TGF-β_(1))、成纤维细胞生长因子23(FGF-23)、生长分化因子-15(GDF-15)水平。结果重构组和非重构组病人的血清PLA2、Hcy、PⅠNP、CⅠTP、TGF-β_(1)、FGF-23、GDF-15水平均明显高于对照组(P<0.05),且重构组病人的血清PLA2、Hcy、PⅠNP、CⅠTP、TGF-β_(1)、FGF-23、GDF-15水平均明显高于非重构组(P<0.05);Pearson分析显示,心室重构病人血清PLA2、Hcy水平与PⅠNP、CⅠTP、TGF-β_(1)、FGF-23、GDF-15水平均呈正相关(P<0.05);多元线性回归显示,血清PLA2、Hcy、TGF-β_(1)、FGF-23、GDF-15为LVESV的影响因素(P<0.05)。结论AMI病人血清PLA2、Hcy的升高对左心室重构具有预测价值且升高TGF-β_(1)、FGF-23、GDF-15水平是其介导左心室重构的可能机制。
Objective To investigate the relationship between serum phospholipase A2(PLA2),homocysteine(Hcy),and ventricular remodeling in patients with acute myocardial infarction(AMI).Methods A total of 96 AMI patients admitted to Kaifeng Central Hospital from March 2015 to June 2018 were divided into remodeling group and non-remodeling group according to whether ventricular remodeling occurred.The healthy people who underwent physical examination during the same period were taken as control group.Serum levels of PLA2,Hcy,N-terminal collagen propeptideⅠ(PⅠNP),C-terminal collagen peptideⅠ(CⅠTP),transforming growth factor-β_(1)(TGF-β_(1)),fibroblast growth factor 23(FGF-23),and growth differentiation factor-15(GDF-15)were measured.Results The levels of serum PLA2,Hcy,PⅠNP,CⅠTP,TGF-β_(1),FGF-23 and GDF-15 in the remodeling group and non-remodeling group were significantly higher than those of the control group(P<0.05),and the serum levels of PLA2,Hcy,PⅠNP,CⅠTP,TGF-β_(1),FGF-23,and GDF-15 in remodeling group were significantly higher than those of the non-remodeling group(P<0.05).Pearson test showed that the serum PLA2,and Hcy were positively correlated with levels of PⅠNP,CⅠTP,TGF-β_(1),FGF-23,and GDF-15 in ventricular remodeling patients(P<0.05).Multivariate linear regression analysis showed that serum levels of PLA2,and Hcy,TGF-β_(1),FGF-23,and GDF-15 were positively correlated with LVESV(P<0.05).Conclusion The elevation of serum levels of PLA2 and Hcy in patients with AMI may be the predictive factors for ventricular remodeling,and the increase of TGF-β_(1),FGF-23,and GDF-15 may be the possible cause of left ventricular remodeling.
