3D打印技术在PHILOS锁定钢板治疗肱骨近端NeerⅢ、Ⅳ型骨折中的应用 被引量:4

摘要 目的分析3D打印技术在PHILOS锁定钢板治疗肱骨近端NeerⅢ、Ⅳ型骨折中的应用效果。方法选取2017年1月至2019年1月我院骨科收治的46例肱骨近端NeerⅢ、Ⅳ型骨折患者,分为观察组(n=23)和对照组(n=23)。对照组采取常规PHILOS锁定钢板治疗,观察组辅以3D打印技术。记录两组患者手术相关情况、术后观察指标、Neer评分及CMS评分,并进行统计学分析。结果观察组在手术时间、术中出血量、C臂透视次数及解剖复位率等指标上均优于对照组(P<0.05)。观察组在骨折愈合时间、肱骨颈干角、肱骨头内翻角、肱骨头高度丢失等指标上明显优于对照组(P<0.05)。术后12个月时观察组的Neer评分及CMS评分均优于对照组(P<0.05)。结论3D打印技术应用于PHILOS锁定钢板治疗肱骨近端NeerⅢ、Ⅳ型骨折,能有效减少手术时间及术中出血量,提高复位质量,促进术后恢复,值得在临床推广应用。 Objective To analyze the effect of 3 D printing technology in the treatment of Neer Ⅲ and Ⅳ fractures of proximal humerus with PHILOS locking plate. Methods A total of 46 patients with Neer Ⅲ and Ⅳ proximal humeral fractures admitted to our department from January 2017 to January 2019 were selected and divided into observation group (n = 23) and control group (n = 23). The control group was treated with conventional PHILOS locking plate,and the observation group was supplemented with 3 D printing. Operation related information,postoperative observation indicators,Neer score and CMS score of patients in the two groups were recorded,and statistical analysis was performed. Results The observation group was better than the control group in operation time,intraoperative blood loss,c-arm fluoroscopy times and anatomical reduction rate (P<0. 05).The observation group was significantly better than the control group in fracture healing time,cervical trunk Angle of humerus,varus Angle of humerus head and height loss of humerus head (P< 0. 05). Neer score and CMS score in the observation group were better than those in the control group at 12 months after surgery (P< 0. 05). Conclusion The application of 3 D printing technology in the treatment of Neer Ⅲ and Ⅳ proximal humerus fractures with PHILOS locking plate can effectively reduce the operative time and intraoperative blood loss,improve the reduction quality and promote postoperative recovery,which is worthy of clinical application.
