摘要 [目的]探讨大转子疼痛综合征的磁共振表现,以提高对本病的认识。[方法]回顾性分析107例临床确诊为大转子疼痛综合征患者的MRI表现。[结果]大转子疼痛综合征主要的MRI表现:在脂肪抑制T2WI及质子加权成像脂肪抑制(PDFS)序列上,(1)肌腱炎和腱鞘炎,前者表现为沿肌肉及其肌腱走行区的高信号,后者表现为沿腱鞘走行区周围分布的高信号;(2)肌腱撕裂,部分撕裂表现为臀肌肌腱内不连续线状高信号;完全撕裂表现为肌腱内连续的线样高信号;(3)转子滑囊炎,表现为极高信号;(4)大转子局部骨质改变,大转子骨质内见局限性高信号灶;(5)髂胫束增厚,表现为不均匀、杂乱的信号。[结论]MRI检查,尤其是脂肪抑制T2WI及PDFS成像可清晰显示大转子周围肌肉、肌腱、关节囊等解剖结构和病变内部情况,对大转子疼痛综合征具有重要的诊断价值。 [Objective]To investigate the MRI manifestations of greater trochanteric pain syndrome in order to improve the understanding of the disease.[Methods]The MRI findings of 107 patients with greater trochanteric pain syndrome were analyzed retrospectively.[Results]The main MRI findings of greater trochanteric pain syndrome on fat suppression T2WI and PDFS images were as follows:(1)Tendon-itis and tenosynovitis,the former showed high signal intensity along the muscle and its tendon,the latter presented high signal intensity around the tendon sheath.(2)Tear of tendon,the partial tear of tendon showed discontinuous linear high signal separation in gluteus tendon,whereas the complete tear of tendon showed continuous interruption of tendon in gluteus tendon.(3)Trochanter bursitis,presented extremely high signal intensity.(4)Local bone changes in the greater trochanter,which showed localized high signal intensity in the greater trochanter bone.(5)The iliotibial band thickening,showed messy signal.[Conclusion]MRI examination,especially fat suppression T2WI and PD images,can clearly reveal the anatomical structures around the greater trochanter,tendons,joint capsule and the internal conditions of the lesions,which has important diagnostic value for greater trochanter pain syndrome.
