摘要
目的将精益管理应用于急诊剖宫产流程的优化,探究其对剖宫产及时率的影响。方法随机抽取2020年1月-2月期间惠州市第一妇幼保健院以常规急诊剖宫产手术流程管理的67例产妇作为对照组,并以2020年4月-5月采取精益管理后收治的45例急诊剖宫产产妇为观察组,比较两组剖宫产及时率、产妇情绪及母婴结局。结果观察组总流程时间、环节总时间、等待时间明显短于对照组,差异有统计学意义(P<0.05)。与对照组比,观察组产妇并发症、出血量>1000ml发生率及新生儿5min Apgar评分<7分、转入NICU的发生率低于对照组,差异有统计学意义(P<0.05)。入院时,两组SAS、SDS量表评分比较,差异无统计学意义(P>0.05);出院时,两组SAS、SDS量表评分均低于入院时,且观察组两量表评分明显低于对照组,差异有统计学意义(P<0.05)。结论基于精益管理理念运用价值流程图,找出急诊剖宫产流程中需改善的重点环节,提高流程的有效性,通过局部的改善来达到全局最优化的效果,提升剖宫产及时率。
Objective To apply lean management to the optimization of emergency cesarean section process and explore its impact on the timely rate of cesarean section.Methods 67 pregnant women who were managed by routine emergency cesarean section in Huizhou first maternal and child health hospital from January 2020 to February 2020 were randomly selected as the control group,Taking 45 emergency cesarean section pregnant women treated after lean management from April 2020 to may 2020 as the observation group,the timely rate of cesarean section,maternal emotion and maternal and infant outcomes were compared between the two groups.Results The total process time,total link time and waiting time in the observation group were significantly shorter than those in the control group(P<0.05).Compared with the control group,the incidence of maternal complications,bleeding≥1000ml,Apgar score<7 in 5 minutes and the incidence of transferring to NICU in the observation group were lower than those in the control group(P<0.05).At admission,there was no significant difference in SAS and SDS scores between the two groups(P>0.05);at discharge,the scores of SAS and SDS in the two groups were lower than those at admission,the scores of the two scales in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Based on the lean management concept,the value flow chart is used to find out the key links to he improved in the emergency cesarean section process,improve the effectiveness of the process,achieve the overall optimization effect through local improvement,and improve the timely rate of cesarean section.
