摘要
目的研究半夏泻心汤对幽门螺杆菌(Helicobacter pylori, Hp)相关性胃炎小鼠叉头翼状螺旋转录因子3(orkhead/winged helix transcriptionfactor, Foxp3)、转录因子孤核受体(orphan nuclear receptor, RORγt)及其相关细胞因子等表达的影响,揭示半夏泻心汤治疗Hp相关性胃炎的作用机制。方法将120只C57BL/6小鼠随机分为正常组、模型组、四联组、全方组、辛开组、苦降组各20只。模型组、各给药组予Hp菌液灌服、自由饮用MNNG进行造模。实验研究分两个阶段,第一阶段为Hp相关性胃炎造模1个月,第二阶段为Hp相关性胃炎造模2个月,每个阶段每组抽取10只小鼠进行相应药物干预及相关检测。正常组、模型组予生理盐水灌胃;四联组予四联杀菌药物灌胃;全方组予半夏泻心汤浓煎剂灌胃;辛开组予辛开汤浓煎剂灌胃;苦降组予苦降汤浓煎剂灌胃。qRT-PCR、Western blot法检测小鼠胃组织中FoxP3、RORγt mRNA和蛋白的表达,ELISA检测小鼠血清细胞转化因子-β(transforming growth factor-β,TGF-β)、白细胞介素-10(interleukin-10,IL-10)、白细胞介素-17A(interleukin-17A,IL-17A)、白细胞介素-6(interleukin-6,IL-6)的表达。结果在第一阶段、第二阶段中,与正常组比较,模型组小鼠FoxP3、RORγt mRNA和蛋白、相关细胞因子TGF-β、IL-10、IL-17A、IL-6表达均升高(P<0.01);与模型组比较,除第一阶段半夏泻心汤全方组的IL-10下降无统计学差异外,余上述指标经四联组、半夏泻心汤全方组干预后表达均下降(P<0.01),辛开组、苦降组也可在一定程度上降低上述指标。纵向观察两个阶段各指标数值发现,随着Hp相关性慢性胃炎的病程发展,FoxP3、RORγt mRNA及相关细胞因子的数值呈增高水平。结论 Hp感染可导致FoxP3、RORγt及其相关细胞因子表达异常,且各项指标随病程发展而增高,可能是脾胃气机升降失常导致"清阳不升、浊阴不降"的病理状态。而辛开苦降之半夏泻心汤及四联杀菌干预后可通过调控FoxP3、RORγt及其相关细胞因子表达,调节免疫微环境,发挥治疗Hp相关性胃炎的作用。
Objective In this study, the expression of Banxia Xiexin Decoction on the stomach tissues of mice with HP-related gastritis was studied, so as to reveal the mechanism of action of Banxia Xiexin decoction in the treatment of HP-related gastritis. Methods A total of 120 C57 BL/6 mice were randomly divided into normal group, model group, quadruple therapy group, Banxia Xiexin Decoction whole group, Xinkai group and Kujiang group with 20 mice in each group. Model group and drug intervention groups were given Hp bacterial solution by gavage and freely drinking MNNG for HP-related gastritis modeling. The experimental study was divided into two stages, the first stage was HP-related gastritis modeling for 1 month, and the second stage was HP-related gastritis modeling for 2 months. Ten mice were selected from each group at each stage for drug intervention and related molecular detection. Normal group and model group were given normal saline. The quadruple therapy group was administrated with quadruple bactericidal drugs. The Banxia Xiexin Decoction whole group was given Banxia Xiexin Decoction and decoction by gavage. The Xinkai group to Xinkai soup thick decoction gavage;The Kujiang group was given bitter drop decoction by gavage. The expressions of FoxP3, RORγt mRNA and protein in the stomach tissues of mice were detected by QRT-PCR and Western blot, and the expressions of serum cell transformation factor-β, interleukin-10, interleukin-17 A and interleukin-6 were detected by ELISA.Results In the first and second stages, compared with the normal group, the expressions of Foxp3,RORγt mRNA and protein, related cytokines TGF-β, IL-10, IL-17 A and IL-6 in the model group were increased(P<0.01).Compared with the model group, the expressions of the above indexes were decreased after the intervention of the quadruple therapy group and the whole Banxia Xiexin Decoction group(P<0.01), except that there was no statistical difference in IL-10 in the whole Banxia Xiexin Decoction group at the first stage(P<0.01). The above indexes could also be reduced in Xinkai and Kujiang groups to some extent. Longitudinal observation of the values of each index in the two stages showed that Foxp3, RORγt mRNA and related cytokines increased with the progression of Hp-associated chronic gastritis. Conclusion Hp infection can lead to abnormal expression of FoxP3, RORγt and related cytokines in gastric tissue.After Banxia Xiexin Decoction or quadruple therapy intervention, the expression of Foxp3, RORγt and related cytokines can be regulated to regulate the immune micro-environment and play a role in the treatment of Hp associated gastritis.
