摘要 目的基于医院信息化环境,构建动态监测、自动提取的压力性损伤监测系统,以实现对压力性损伤的实时全面监测。方法依据临床实际工作流程,初步确定压力性损伤相关信息来源及筛查策略,通过临床调研病例信息优化筛查策略,确定压力性损伤疑似病例筛查策略形成疑似病例数据库,并经临床确认纳入病例数据库,构建了实时监测系统。结果压力性损伤筛查策略实现了数据自动获取;实时监测系统对住院患者压力性损伤的总检出率为90.00%,提高了压力性损伤的病例报告率。结论压力性损伤实时监测系统突破了护理质量管理中的难点,制订了真实、客观、科学的筛查策略,可有效检出压力性损伤事件,有助于规范压力性损伤的护理管理流程。 Objective To construct a dynamic monitoring and automatic extraction pressure injury monitoring system based on hospital information environment,so as to realize real-time and comprehensive pressure injury monitoring.Methods According to the actual clinical workflow,preliminarily determine the information sources and screening strategies related to stress injury,optimize the screening strategy through clinical investigation case information,and determine the screening strategy for suspected cases of stress injury to form a suspected case database,which is clinically confirmed and included in the case database.A real-time monitoring system was constructed.Results The pressure injury screening strategy achieved the automatic acquisition of data.The total detection rate of pressure injury in hospitalized patients was 90.00%by the real-time monitoring system,which improved the case reporting rate of pressure injury.Conclusion The real-time monitoring system of stress injury breaks through the difficulties in nursing quality management and formulates real,objective and scientific screening strategies,which can effectively detect stress injury events and help standardize the nursing management process of stress injury.
