摘要
目的探讨丙种球蛋白联合大剂量甲泼尼龙治疗小儿重症病毒性脑炎的临床价值方法选取2019年5月—2021年6月就诊于该院的132例重症病毒性脑炎患儿纳入该研究,按随机数表法分为对照组(66例)和研究组(66例)。两组患儿均给予常规对症治疗,对照组在此基础上给予大剂量甲泼尼龙,研究组给予丙种球蛋白联合大剂量甲泼尼龙治疗。比较两组临床疗效、症状消失时间、免疫功能、神经功能。结果研究组治疗总有效率(93.94%)比对照组(81.82%)高,差异有统计学意义(χ^(2)=4.552,P<0.05)。研究组发热、抽搐、意识障碍、颅神经受累、脑膜刺激征消失时间比对照组短,差异有统计学意义(P<0.05)。研究组治疗后IgM、IgA、IgG均比对照组高,NGF、NSE、S100β水平均比对照组低,差异有统计学意义(P<0.05)。结论丙种球蛋白联合大剂量甲泼尼龙治疗小儿重症病毒性脑炎的临床疗效确切,可缩短症状消失时间,调节免疫功能,提高对神经功能的保护,临床应用价值高。
Objective To explore the clinical value of gamma globulin combined with high-dose methylprednisolone in the treatment of severe viral encephalitis in children.Methods 132 children with severe viral encephalitis who were admitted to the hospital from May 2019 to June 2021 were included in this study,and were divided into a control group(66 cases)and a study group(66 cases)according to the random number table method.Both groups of children were given conventional symptomatic treatment,the control group was given high-dose methylprednisolone on this basis,and the study group was given gamma globulin combined with high-dose methylprednisolone treatment.The clinical efficacy,time of symptom disappearance,immune function and nerve function were compared between the two groups.Results The total effective rate of treatment in the study group(93.94%)was higher than that in the control group(81.82%),the difference was statistically significant(χ^(2)=4.552,P<0.05).The disappearance time of fever,convulsions,disturbance of consciousness,cranial nerve involvement,and meningeal irritation in the study group was shorter than that in the control group,the difference was statistically significant(P<0.05).After treatment,IgM,IgA,and IgG in the study group were higher than those in the control group,and the levels of NGF,NSE,and S100βwere lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion Gamma globulin combined with high-dose methylprednisolone has a definite clinical effect in the treatment of severe viral encephalitis in children.It can shorten the time of symptom disappearance,regulate immune function,and improve the protection of nerve function.It has high clinical application value.
